Seriously though....Justin "Hawk" Bradshaw to Acolyte Bradshaw to JBL is a damn drastic one. Was just gonna mention that one. When I first got back into wrestling in 05 or so, I had no idea that JBL was the same guy as the Bradshaw I knew from the APA.

I recall seeing some in the PWI 25th Anniversary issue. The one that made me do a double-take was Wolfie D. He went from "street hood"/Scotty II Hotty-like in his gimmick to one of a sinister cult figure with a bald head and tattoos all over.

back in 1997 i instantly recognised that Nash & Hall were Diesel & Razor, but it took me awhile to figure out syxx was the 123 kid.

I don't think we should consider masked wrestlers who changed gimmicks (i.e. smash to repo man) because it's hard to tell what they looked like to begin with. Or, in cases like diesel-to-kane, there's no way of knowing that the former diesel was under the red mask.
I think the best examples include sting (surfer to crow), bradshaw, superstar graham, isaac yankem to diesel, and things like this.

What about this version of Snitsky? To this one: There was also a bit of a change in character, even though it could be argued. The 2004-2005 version may have been an unstable-like character, but in early 2006, he kind of had some comedic moments with Goldust and such. However, the 2007-2008 version, while very unstable (Again), he had the whole monster thing going on.

I think that it's because I grew up watching mostly WCW but it's still bizarre for me to think that "Stunning" Steve Austin and "Stone Cold" Steve Austin are the same guy.

It was just as stunning for me to see Marcus Alexander Bagwell, the guy I'd last seen as a dorky Two Cold Scorpio dancing babyface in '93, was strutting around looking like an oiled-up male stripper when I started re-watching around '97.
I came here to say Alex Wright-Berlyn and Surfer-to-Crow Sting, too.
