I never connected Cactus Jack with Mankind until the WWE outright brought it up themselves.

Rockers Shawn Michaels and 2011 Shawn Michaels barely look like the same person. Sure, it's been over 20 years, but some guys barely changed in that timeframe.

Odd moments of realization: -"Hey, didn't this Berzerker guy used to be a lumberjack?" -I caught onto the fact that Repo Man was Demolition Smash, but it wasn't until around that time that I also figured out he was Krusher Kruchev. -During the entire time that Foley was creepy Mankind, I never figured out he was Cactus Jack, though I never saw his introductory vignettes until much later. -"Since when was Scotty the Body depressed and grunge?" -I don't know if I ever would have figured out on my own that Berlyn was Alex Wright. -Same with Marlena/Alexandra York. -I saw several Goldust matches before I figured out it was Dustin Rhodes. I blame the lack of a mullet. -Same with Big Bossman/Big Bubba Rogers. -"Wow, The Handsome Stranger(Bagwell)turned out to be a serious d**k!" -Upon hearing that the juiced up, bleached blonde guy spouting nonsense catchphrases was Scott Steiner, I was speechless.

The whole hair thing with Austin/Snitsky/Kane/Berlyn/Desmond Wolfe is pretty much due to natural causes. Look at the guys shortly before the change and you could see their thinning hair (well not Kane for obvious hairpiece reasons), so they decided to do something about it and adjust their looks.
Sting was gradually changing into Crow Sting. He stopped bleaching his hair, letting it grow out and started to put on less colourful facepaint. Then he was gone for a little bit and even after returning still worked on finalizing the Eric Draven look.
However, I recognized all of those mentioned as who they were before. Not so much with Barry Darsow. Repo Man looked so much different than Demolition guy (don't remember who he was) or Blacktop Bully or whatever other gimmick he had. It also took me some time to realize that Goldust was Dustin Rhodes. The Disciple also looked so much different (better, younger and in so much better shape) than Brutus Beefcake/Zodiac/Booty Man/Man With No Name.

Scott Steiner's gradual change from "amateur-background looking guy with a curly brown mullet (and, at one point, the worst pink singlet ever to grace a TV screen)" to "Genetic Freak Booty-Daddy Holla If Ya Hear Me" is pretty drastic in hindsight.
Took me a little while to catch on that Goldust was Dustin Runnels under the paint and gender-bending, too.
