Seriously, I'm terrified that something's going to happen to him if he keeps nullifying every single viable opportunity that comes up to write him off television for even a little bit.
There's been seemingly dozens of chances to give the man a break over the past few years that he keeps effectively no-selling... Off the top of my head, there's been...
- Nexus Firing - CM Punk/McMahon Firing - Crisis of Character after Losing to The Rock - This "Vacation"
And I feel like there's been even more than that. I'm increasingly getting a Kurt Angle "dangerously addicted to competition" or being the company man vibe from him. And you can see it just looking at him. He's seriously close to getting presidential amounts of aging in the same time frame. Look at the man now and compare him to 2003. Even compared to his contemporaries he's visibly aged more, and if he keeps up this nonstop motion he might wind up in an even worse early boat than Edge. I'm seriously worried for the guy's long-term health and well-being.
Just why does he keep passing on the breaks?

The way he talks about fighting for the boys in the back, he probably worries that they won't get much exposure without him around to draw.

I honestly thought he was gone after last night's promo. HGH is a helluva drug. Anyways, I just hope that he doesn't end up dead in his hotel room due to exhaustion among OTHER things....

At the rate he's going I can see him retiring early, maybe within the next two years. Of all the occasions listed in the OP though, it really should have been the Nexus situation. That leaving promo was really quite poignant, and then after getting just about everybody on his side, he turns up every single week as usual. Wade should've won the title at TLC that year, and then Cena should've returned at the Royal Rumble to set up Barrett vs. Cena for Wrestlemania 27.

We don't know if WWE offered him any. In fact, WWE could force him to take a break in an instant if they wanted. Seems to me that the more likely scenario is that WWE isn't offering him one, not that Cena is refusing to. ANd he isn't "effectively no selling" this one. Kind of hard to do so when your arm is in a sling and Johnny L just tried to break it off.

2003 WAS nine years ago, mind... I guess I meant that, in the right lighting or right angle, as seen from that infamous "Angry Face" gif, Cena looks a LOT older than the nine-year difference would suggest, especially when compared to people like Orton that came aboard at the same time more or less yet don't look nearly as visibly aged.

It seems we're mixing kayfabe and real life here. Those were all storylines, not planned breaks that Cena refused.
I think Cena doesn't want to take time off, because he's the top dog and doesn't want to give up his spot. Totally natural, and if you're a badass like that and can do it all, do it all. You only live once.
