Back when I was in National Guard a guy brought in his beastiality tape. I've never watched anything like that before or since. Most of it wasn't shocking but there was one scene involving "fluids" that I had to look away from before I hurled.
Regular movies have made me feel uncomfortable before, the original Last House on the Left comes to mind, but I've never hurled.

Came damn close when viewing A Serbian Film and was even closer when viewing Salo. I really need to stop letting the Cinema Snob peak my interest when he mentions disgusting movies. I will also add The Fly (Goldblum version), and The Human Centipede Part 2. Never threw up...but it is very hard for me to eat anything lumpy the second those movies pop into my head.

I came pretty flippin' close the first time I saw "Nekromantik".
"Johnny Got His Gun" was another. I'm not quite sure how to explain it, but it left me feeling very claustrophobic and queasy.

Just the thought of internal isolation (of the highest magnitude) makes me uneasy and frightened. Great movie and book, but damn does it stay with you forever Human Centipede 2 is the closest a movie got me to throwing up. Majority of the film, I was ok with. But then came the scene where {Spoiler}The "doctor" starts cutting open their knees and cutting off their tendons/muscles . Really shouldn't have watched that movie while eating fried chicken

I hardly ever throw up for any reason, so no.
A former roommate of mine had to do everything in his power to choke it back down after the "something's wrong with meeeeeee!" scene from Slither, though.
Personally, I felt somewhat sick after watching Mysterious Skin, but not nauseous. More like someone had just... I don't know... violated... me... somehow.

Movies don't really ever get to me like that, but that finger-peeling scene in Black Swan really made my skin crawl.
On a somewhat related note, there are a few cartoon scenes that are worse for me than anything live-action. Besides a lot of Ren & Stimpy stuff, the Spongebob episode about the splinter in his finger and the scene with Squidward's toenail are channel-changers for me.

I never have puked, but it was amazing to watch all of the people in the movie theater during "Cloverfield" run up the aisles. My son started getting a little queasy so I told him to shut his eyes.
I will say that damn movie made me jump for the first time in a long while when the monster's head popped up during the subway scene.
