David Carr had a chance to be great. Unfortunately, they never gave him an offensive line to protect him and he got so twitchy that it forever ruined his shot at being a starting QB.
Unfortunately, it seems like Sam Bradford is having this happen to him as well, with the added problem that they keep changing the offense on him.

I'll have to say Scott Gomez is pretty unlucky. He'd decided to coast for the rest of his career but he ends up getting sent to the worst place in the NHL to do it. We're hyper-critical of our players here (people would bitch about a 50 goal scorer if we actually had one now)and him being here must be a total blast for him.
DiPietro is so unlucky that it makes you wonder if he's got some curse put on him considering how his career has gone. There are people who have bad luck and then there are people like DiPietro.

Kerry Wood and Mark Prior can trace their declines to Dusty Baker.
Here's one, Ki-Jana Carter. On his 3rd carry of his first game in the NFL (and it was pre-season), his knee gets destroyed. He never recovered, and made good on the promise he had.

How about Frank Thomas? Yes, he'll go down as one of the best hitters of all-time and a hall of famer.
1st in 1994, the Sox are having a great season. A great lineup with Ventura, Julio Franco, Ozzie Guillen, Tim Raines, etc.
A great pitching staff led by defending Cy Young winner Jack McDowell.
He's the reigning MVP and would win the award again that year.
The White Sox look like the best team in baseball, and what happens? The God Damn Strike!
In 2005, The Big Hurt would finally get his ring after all those years, but he would be injured pretty much the whole season and wouldn't contribute.
I don't know what's worse. Putting your blood, sweat, and tears into the game and not getting a championship? Or the team that you were the heart and soul of for over a decade finally winning the big one with you injured all year and not playing.
It would be like if LeBron or Kevin Durant went their whole careers without winning a title, only for their team to win one after they tear their ACL.

In Scotty's case, I think you're confusing unlucky with lazy.

Here's a hockey player I know triplethreatmark considers more unlucky than lazy- Martin Havlat.
When he was healthy- a big emphasis on when- he had such skill and talent he stood out on the ice. In 2005-06 with Ottawa, he played in just 18 games but had 9 goals. Over a full season, he'd have had 41 goals.
Also very unlucky in that the Senators made the Stanley Cup Finals the year after he left Ottawa, then the Blackhawks let him go to Minnesota the year before they won the Cup.
