Holy s*** I completely forgot Flash Forward existed! I can't even remember what happened but I watched every goddamn episode. What happened?! I seem to remember they left us on a cliffhanger and then the show got cancelled so we never got any answers. I really liked the show though and was disappointed it got cancelled. Plus, it had a badass soundtrack. Lost was the example of this for me. First few episodes were good, then it got confusing, then it got stupid, then it got beyond ridiculous - and that was just the first season. Also Two and a Half Men. First couple of seasons were good, 3-5 were awesome then it all went downhill. People can blame the decline on Ashton Kutcher (not denying it got even worse when he came in though) but it started long before he arrived.

'24'. Jack Bauer gets captured by Chinese and shows back up in a Tom Hanks 'Castaway' beard.
Then his dad shows up and some more Jezebels infiltrate CTU for the hundredth time, and Tony Almeida shows back up from the dead and more ridiculousness occurs.
The show was never realistic, but damn, they ran out of ideas after the 4th season.

Married...With Children got to be so painful to watch, I barely remember a handful of episodes the last 2-3 seasons.

This should begin and end with Roseanne. Great first few seasons but then everybody became a lesbian, Becky left, Darlene and David got married, and they won the lottery.

Another one that got real bad was 'Sanford and Son', especially when they went to Hawaii.

Hard to say the show fell off a cliff when the viewer average jumped by 5 million and never dipped more than a million less than that. it still fell off a cliff, regardless of how many gumps kept watching it. When a show falls off a cliff, viewers leave and never return. The show becomes universally disliked and the majority find it to be unwatchable. That's why Heroes is the go-to response. That wasn't the case with Friends. You may have stopped liking it, but the majority of people still found it to be overwhelmingly entertaining. It didn't get anywhere near the cliff.

"One on One" in the fifth season when Breanna and Arnaz move to LA. It became a show about them and the new people with Flex appearing every so often. It felt like a different show after Breanna and Arnaz graduated.
Also, "New York Undercover" in the fourth season. The only people who were still around were Williams and Moreno. They had killed off two main cast members, another one was gone (Well, she was the captain, so Williams and Moreno didn't work at that precinct anymore) and were replaced by three other people. One of them being Tommy from "Martin." I am going to be honest, I kind of enjoyed the reruns of the fourth season, but I was mainly watching them for the villain who killed off the two cast members in the third season finale.

I think the creator of any show should have an ending in mind before the pilot even airs. It sucks to see a show go downhill-it's like have a terminally ill person on life support

Torchwood S1 - Started at the bottom of the cliff surrounded by sharks, it was meant to be "adult" but seemed to be written by 14 year olds. S2 - Clawing it's way up the cliff, this is actually pretty good. Children of the Earth - Holy s*** this is amazing! It's made it to the top of this metaphorical cliff. Peter Capaldi stole the show. Miracle Day - Oh dear it's fallen of the cliff again, to be fair it was a good premise but they stretched about 3 episodes worth of material over 10 episodes. And that is symptomatic of most of the shows mentioned in this thread, many should have been one series stories (Heroes, Homeland) and all of them have been dragged way past their natural conclusion (Dexter should have been caught in S5). I never really liked Torchwood. I watched the first two seasons because I was massively into Doctor Who and loved the Jack Harkness character, but the first 2 never did anything for me. Disliked or hated all of the characters, and my opinion on Jack eventually soured as well. I made it to Children of Earth, and was really hype about it because of how everyone loved it and said it was the best part of the show. It was actually rather good...until they revealed the twist behind the abductions. That twist ruined everything about the season for me, and I might even go so far as to call it "insulting". I then said "f*** Torchwood" and have never given it a second glance.

Some people might refer to it as more of a gradual decline in quality, but I consider the fall of "The Office" to be a sudden and drastic disaster.
I point to the end of The Michael Scott Paper Company. Once that story arc concluded and (the great) Charles Miner departed the show, it had no reason to exist any more. Michael knew he was infallible, there was no great villain and everyone appeared to be happy with one another, totally eliminating any sort of conflict the show may have had.
The show went into an almost immediate nosedive following those episodes in season five. Sure, there have been a handful of good episodes since then, but overall the show is almost unrecognizable when compared to the first four seasons.

House is the epitome of dramas for this. The first three seasons were great (I'm noticing a pattern in that regard) but after that the show just got worse. The straw that broke the camels back for me when the entire original team left and House got a new group of people, I barley watched after that because the story lines were stupid, the diseases were laughable, and Hugh Laurie seemed like he was just playing the character for laughs.
I also have to say Power Rangers after Disney bought the show. Ninja storm was good, Dino Thunder was AWESOME, SPD was meh, and ever since then the seasons have just gotten worse (except for RPM which was awesome). Hell, even when Saban bought the rights back it didn't get better. I gave up on Samurai after the sixth ranger showed up because it was so bad.

The official answer should be and always will be Heroes. I mean...WHAT THE HELL HAPPENED?!?! Season 1 was so damn awesome, and then...yeah! Seriously. Writers strike did not help season 2 but the big problem for them was their inability to kill off a certain villain. Stuck with it in part for 3 seasons, what the hell was the forth?

I am going to mention both Heroes and Glee. Glee has mostly become the Lea Michele show making everything about her and less about everyone else.

I don't know if it was Family Guy right after it came back, it still had some ok episodes. I think the first Conway Twitty thing was the moment the show went Willie E. Coyote.

See, I like Family Guy and all but I think the show's big problem is that they do so many things like the Conway Twitty segments just to spite fans that hate when they do that. I mean, I know that Seth MacFarlane is great at what he does and all, but it's a bit dickish.
