Batista was always leaving sometime in the spring to start promoting Guardians of the Galaxy. The one-two punch of his return being met so poorly and GotG coming out of nowhere to be the year's biggest hit is what kept him from coming back.

I think the plan was for Bryan to hold the belt until Summerslam and then drop it to Lesnar. Bryan's injury led to them hitting the Cena button.
I think the Shield breakup would've happened regardless. But If Reigns hadn't gotten injured, Ambrose and Rollins might not have risen as high up the card as they have. Their feud was going to be a random upper mid card feud while Reigns chased the "bigger threat" of Orton and HHH.

Cena wins. That is always the plan.

As for The Shield, I don't think it would've happened as quickly as it did had Bryan not been injured. I think they would've lasted until at least SummerSlam before Seth turned heel.

It seems like Batista was taking the title at Mania and probably keeping it through his promotion of GOTG. H'ed probably then lose it to Lesnar. I wonder if they were planning on him also playing an absentee champion for a short time during that period too. Though if that was the case, I have no idea what their long term plan with Bryan could be. There's talk of a match with Sheamus (FFS!) at Mania. But who knows where the hell would he go after that.
Granted, this company only seems to think a month or two ahead at a time even for their top guys. So I doubt they had any concrete plans for anyone outside of Cena, Lesnar, and likely Batista.

I think i read somewhere that the original plan was for Batista to beat Orton at WM, then lost it to Cena at SummerSlam.
