Kane comes out right before the match starts as all four men are in the ring. (Or assuming a face scores a pin over someone other than Rollins)

It'd make for some cool scenarios, like a brief Shield reunion to take out Orton, or a psuedo tag match with Orton/Rollins vs. Ambrose/Reigns

I would like that as well. It gives Rollins less of a chance to get a slimy, undeserved win though.

The only way it would make sense is if Rollins scored the first pin. That way, Kane's clearly trying to stack the odds against Rollins to make sure he don't leave with the title. Without that, it comes off like an unnecessary way to extend the match.
I can think of another flaw as well. You really want to pin both Orton AND Roman when it's already kinda slim pickings for Rollins' future title opponents? Granted, a four way would provide them a bit of an out but I'd rather not beat Roman again if they are set on still pushing him as a top guy unless you want him to be the new Ryback where he loses every title match. Orton's already made so it don't really matter if he gets pinned but Roman's much more vulnerable. I don't even really care about the dude but the booker in me keeps focusing on that.
So, I think it could work but, in this particular case, a one fall match is a smarter route to go.

I'd mark for Rollins pulling the Edge move of laying around ringside pretending to be hurt until he can sneak in and steal a pin at the end.

It would be really telegraphed, because there's no way this happens if the first pin occurs at 10:57 ET, but if the first pin occurs at 10:40 ET, you know something's up.
