Kind of an offshoot of the sad music videos, which mentions Freddie Mercury's farewell appearance.
I'll start with mentioning Gorilla Monsoon's last public appearance at Mania XV. You knew something was up.

I'd comment Chris Farley on Saturday Night Live in 1997. Lorne Michaels asked for that episode to never be aired again.

Tammy Faye Messner/Bakker....just do an image search on Google for her, she looked frail as all hell and you could tell she didn't have much time left.

The last few episodes of Siskel and Ebert when you could see Gene's body just not keeping up with his mind.

Eddie Guerrero's last match was against Mr. Kennedy.

The first thing that came to mind was Gorilla Monsoon at WrestleMania XV. It just broke my heart to see him looking so ill.

There was a video of one of Johnny Cash's last public concerts at the Carter Family fold shortly before he died and his son(I think) had to walk him onstage, sit him down, and literally put the guitar in his hand and shape his fingers in the E chord so that he could play

If that last appearance was Tim Curry's last public appearance, it'll be up there.

Burt Reynolds recent appearance somewhere. Looked very frail. Hate to say it, but he cannot have long left.

Jason Robards in Magnolia, playing a man dying of cancer as he himself was dying of cancer.

I'd comment Chris Farley on Saturday Night Live in 1997. Lorne Michaels asked for that episode to never be aired again. His family was very upset he was allowed to host given the shape he was in at the time. I think Lorne regrets not booking some else early in the week now but I can understand his denial. Having seen Bulushi go through the same thing he didn't want to admit it was happening again.

Mr Football Ted Whitten was dying of prostate cancer. About a month and a half before he succumbed before a now defunct AFL State Of Origin match at the MCG he in the back of convertible did a rather enthusiastic lap of the field. But the poor guy was blinded by the disease and needed his son to tell him what part of the ground they were up to..
Happened 20 years ago.

Some of the last pictures taken of Rock Hudson were very sad. Although as it was possibly the first time the world saw the effect of AIDS on a celebrity, his case brought the disease to the attention of a lot more people and whilst tragic, probably raised awareness of the disease.

As mentioned, his last appearance before a crowd was even more sad; but this was like a video goodbye.

After realizing it's been six years since his passing, Michael Jackson. Whether you consider his last appearance to be the This Is It announcement or the rehearsals, the man had so much going on and everyone was eager to experience it. That's what makes it sad to me in addition to the shock of it.
Warrior is another one. Everything was going good for the man and he just buried the hatchets with loads of people. Then, he passes after a weekend of celebration for him. The previous night you saw how out of breath and disoriented he looked, but you wouldn't think 24 hours later he would pass away.

Charlie Chaplin's last U.S. Appearance. He lived another five years but was basically banished from the states.
