Well, if we're looking for the "first"... Evan "The Strangler" Lewis ended up just being known as "Strangler Lewis", at least until Ed came around a decade later. Him going by such a similar name in tribute actually muddied the waters and caused confusion, taking away from the legacy of his idol. Now when people say "Strangler Lewis" they need to quantify.

There was that guy who was originally going to be called Mason the Mutilator, then Mankind the Mutilator. They dropped the second part before he ever debuted, luckily for him.
It's weird that the WWE does this, now, and I don't really get the thinking behind it. Imagine if we'd been left with all those feuds between two guys just called 'Orton' and 'Cena'.
