Like how Razor Ramon pinned Rick Martel for an IC title run despite Martel never being IC champ. Or more recently when Kevin Owens pinned Tyler Breeze for the IC belt.
It'd be cool if we could make a complete list.

And of course there's David Arquette pinning Eric Bischoff to take DDP's WCW Championship.

Psychosis lost the IWGP Junior Heavyweight title back to Jushin Lyger. it was actually Juventud who won it but he got injured and NJPW never agreed to allow the title change to happen in the first place, which killed any working relationship WCW might've had with NJPW.

Owen hart pinned Goldust dressed as Triple H and won the European Title even though the real Triple H was champ.
Does that count?
If not Rock pinned Vince at KOTR 2000 and won the world title.

I believe Savio Vega lost an IC title he never held as well.

Chyna pinned Trish to win Val Venis' Intercontinental title at SummerSlam 2000.
I guess there are a lot of triple threat/fatal four ways that this might be the case.

He claimed Booker gave him permission to defend the TV title while he was injured. I believe he lost it to Jericho, after the Giant interfered.
