2.1997 in amarillo after a match with Bret Hart.
3. 1998 after a tag match with Bradshaw.
4. 2005 after ecw reunion show.
5. 2012 after a tag match with Dreamer.

The thing about 1997 is that only a few months did he appear in the WWF as Chainsaw Charlie and then reverted back to being himself. He was only there from I believe January to July of 1998, and then appeared in ECW later that year, but I don't think he wrestled.
