My vote goes to Kobashi's chops. His opponents usually look like the skin on their chest has busted open, or at least close to it.
What else probably hurts like hell for real?

The Figure Four Leglock hurts like hell if the user puts pressure on it.

I'd imagine a lot of various powerbombs over the years. The Batista Bomb, The Last Ride, and Vader's all looked like hell to take.

Specifically? I can't imagine blading feels too damn good, but I wouldn't know because we had such a prohibitive no-blood policy that I wasn't even taught how to do it. I personally once crotched myself for real in the corner and that wasn't fun. I teared up, I felt it in my guts, felt like I was gonna literally shit myself. And yet, pride being what it is, I answered my opponents' question of "OK?" with a very pained and labored "Superplex" and... he superplexed me. And we went from there!
In general, depends who does what. Sometimes, as in the Kobashi exemple you give, it also depends on intent and how much the receiver is willing to take. Kobashi wants his stuff to land, and his opponents are willing to take it, or at least willing to stay in the match after taking it.
I threw thousands of powerbombs: The keys are 1) Put them in position to have an easy flat bump, you don't want the whole weight of the body landing on ANYTHING 2) If you take it, tuck your head. It's easy, doesn't hurt. Powerbombs that ended in injury (Vader on Thurman, D'Lo on Droz, Sid on Nash...) were accidents, if you do them by the book and nobody slips, then it's real safe.
You need to be in there with an asshole for any submission to hurt for real. OR, you need to be a particularly bad seller and the guy felt you needed motivation to sell better.
You couldn't pay me enough money to give or take one of those bumps on the sides of the ring or on the ring steps.
Some people are just lousy workers and they'll deliver potatoes by the dozen. Others are just as lousy workers and you wouldn't believe their stuff to be real even if you were blind because it's so obvious it doesn't hurt: The strikes come no where close to landing, and they don't sell that they are pulling on whatever they are supposed to be pulling on or putting pressure when they have you in a hold... Neither of them is good: Either you are hurt physically and you get distrust-y of the opponent, or the match gets hurt due to having to sell fake looking shit.

Most basic moves hurt if applied legitimately. Stuff like wrist locks all have basis in legit submission fighting so it doesn't take much to make a worked version hurt if needs be.
