Can someone explain that one to me? It wasn't as if he wasn't over as a face back then and it just seemed such a waste after that epic face turn of his against Razor and Flair just 2 years earlier. It didn't even serve a purpose as he was gone soon after that and never worked a program with Luger.

Sure, but he was supposed to work a Luger program, so the heel turn made sense, it just wasn't followed through on.

I remember being a kid at WM X, and I was so confused by the ending of the Lex/Yoko match.
Like, confused because it was done very strangely, but also because I didn't like "Made in the USA" Lex and really liked Mr. Perfect and was pumped that he was back, so I had no intention of booing him.

The Luger/Yoko feud was wrapping up.
Just looking at other heels he might have worked with at the time:
1. Owen - feuding with Bret 2. HBK/Diesel - feuding with Razor 3. Lawler - part time feuding with Bret 4. Crush - imagine how horrible those matches would have been 5. Bam Bam - might have worked well with Luger 6. IRS - by this time a lower midcarder on his way out
So, it would have made 'perfect' sense had the feud actually played out. Perfect would have likely been the best choice to get some pretty good matches with Luger.
Babyface Perfect vs. Narcissist would have been even better, but they f'd that up by turning Lex face.

Yeah I remember they were hyping it up for the Wrestlemania Revenge Tour and I think Crush ended up replacing Perfect and they went with a feud with those two instead.

Perfect broke his toe in a home accident--supposedly--and pulled out of the planned matches and just never came back (for a year and a half, at least). In actuality I think he just decided that they weren't paying him enough for it to be worth it.
Honestly, with Perfect as the referee, I'm not sure there's any other possible finish--based on their history--besides him f***ing Lex over. It's one of the reasons I never bought into the "Luger blabbing the WM finish and getting it changed" story, even before it was more thoroughly debunked. The identities of the two guest referees was the worst-kept secret in wrestling--I was an adolescent with a Prodigy account and I knew who the guest refs were going to be weeks in advance--and given the history between not only Lex & Perfect but Bret & Piper, it telegraphs the planned sequence more than you might think.

According to Luger, Perfect blanked out at WM 9 and told Luger in the middle of the ring he forgot what to do next. Luger said most likely he was ribbing. Now that Perfect is dead, it will forever remain a mystery.

I'm not aware of the Lloyd's policy having anything to do with this. Why would they push him otherwise? I believe he took the insurance payout for a career ending injury. Then decided to return, so he had to pay back part of the settlement. I forget exactly.

I seem to recall WWE paying off Lloyd's of London for his comeback at the end of 1992. When he left in 1993, I think he claimed that he was retiring and cashed in again because supposedly someone in WWE's front office told Lloyd's that Hennig was preparing to wrestle for them again in 1996. (Of course, he sat out his deal and wrestled in WCW instead the next year. Not sure whether Hennig or WCW had to pay Lloyd's off the second time.) By that point, Lloyd's stopped issuing policies to wrestlers because too many guys took advantage of them.

I believe he preferred being heel and I guess making him heel and promising him a top program with Luger was a good incentive to get him back after he walked out in November.
Perfect vs Luger would have been the No.2 program behind Bret vs Owen.
