It's been 20 years since a face won and it looks like no face will ever win at this rate.

Don't heels generally win most tournaments?
Anyway, it's because they insist on doing the whole king gimmick after the win I think that forces it to be a heel. Winning it just makes you look like such a douche if you put on a crown and cape.

It seems to counteract how few heels have actually won the Royal Rumble. Don't a lot of MITB winners tend to be heel or become heel immediately after to balance that out?

Looking those stats up, of the 24 Money In The Bank matches there have been, there have been 14 heel winners and 10 babyface winners. But of those babyface winners, four of them (maybe five depending on how you interpret Strowman last year) used their cash-in to help develop a heel turn (Punk in 2009, Kane in 2010, Bryan in 2011 and Orton in 2013).

A heel winning KOTR is almost as predictable as the winner adopting an over-the-top king gimmick.

I didn’t count Bayley as a face winner turning heel as her actual turn was long after her cash-in and came as a result of an unrelated angle. And Orton was still babyface until he cashed in on Bryan at SummerSlam 2013. Kane was a tricky one, given that he was right in the middle of the angle that was eventually turning him heel, but he hadn’t actually turned yet.
