And now he has the world title again. Has anyone had this huge of a gap between their first and last world title reigns? I’m sure flair maybe in wcw, but I can’t think of anyone else

I’m sure flair maybe in wcw, but I can’t think of anyone else Hogan won his first world title in 1984 and defeated Triple H for his last WWE Title in 2002. So not exactly two decades, but close. If you include AEW, Jericho's first reign was in 2001 and he won the AEW Title in September 2019. If we're being technical, I think this run for Lesnar is 19 years and 4 or 5 months since his first reign.

There's Thesz as well but his is harder to say in terms of what you'd class as a world title nowadays.
American Wrestling Association (Paul Bowser in Boston, not Verne Gagne) and MWA (Ohio) world titles - 1937 National Wrestling Association title - 1939 (I'd consider this one a definite world title) His final National Wrestling Alliance title - 1963 American Wrestling Alliance (again, not Verne) world title - 1964 Worldwide Wrestling Associates (this would go on to be NWA Hollywood Wrestling) world title - 1966 UWA World title - 1977
Depending on your definition of world titles, Thesz is around 40 years difference.

That ship very likely sailed with Hunter’s health problems last year. We will be lucky if he sees another match again, much less a title reign.

Would that make his last match the tag on the Saudi show? No. There was that Triple H vs. Randy Orton "fight" on the January 11, 2021 Raw that didn't have a bell, referee, or finish that wasn't Fiend nonsense. However, it did have entrances, bumps, and was hyped throughout the show. As far as official matches go, Cagematch has a couple matches from a Japan house show loop in mid-2019, those matches being Triple H and The OC/Club vs. Corbin, Lashley, McIntyre, and Samoa Joe, and the next night his final official match was Triple H and Nakamura vs. Roode and Joe in Tokyo.

Um, not a full 20 years yet. Give it until August and then we have a full 20 years.
