What are some of the creepiest imagery you have ever seen in horror movies?
When the shark in Jaws approaches the man in the pond underwater, it is an image bad enough to give you nightmares.
Another is the spheres coming through the air towards people in Phantasm.
Friday the 13th is rarely scary, but Jason on top of the crashed RV in part 6 conveys so well Jason's indestructability.

Not movies, but some manga examples.
Just... body horror in general. Like Uzumaki. People morphing into some twisted giant snails, etc. There's also a chapter in Kurosagi Corpse Delivery Service where a snail parasite, who infects the snail to make its eye stalks swell up to make it more noticeable to birds, infects a human. Saw it once like around a decade ago, still hangs out in my mind to creep me out. And by creep me out, I mean gives my mind a few second to go AAAAAAAAAAAAAAAAAAAAAAAAAAAAAAAAAAAAAAAAAAAAAAAAA
In general, I'm less creeped out by displays of gore and mutilation and more creeped out by transformation. Not dying horribly, but morphing into something horrible/

Every time I see a Slenderman cameo in a video game. I'm playing Phasmophobia solo so I'm already quite on edge, I do NOT need to think I see the lanky bastard off in the trees somewhere!

There are absolutely creepier scenes and imagery that will come to mind for me after I finish posting this, but one off the top of my head is in The Strangers when you see the guy lurking in the background of the kitchen and the main character doesn’t notice.

Another good one is the unfortunately never finished comic Bedlam by Nick Spencer. Takes the concept of totally-not-The-Joker going sane and plays it as horrifying as humanly possible

The House On Haunted Hill remake, when Ali Larter looks through the camera and sees the old timey doctors, who look up at her.
Then she looks away and the monster comes at her with the slow/fast Jacob's Ladder effect.
Found the scene incredibly unsettling.
