The twins in Streets of Rage 1 were more annoying than tough, but that was because it was hard to time an attack on them. Also, Sodom in the first Final Fight was hard to beat at times.
Also, let me talk about the Def Jam games, if these types of games count in this discussion. In Def Jam Vendetta, I remember thinking the boss characters (and they were the rappers) were tough, like N.O.R.E. and Ludacris, but DMX, along Method Man & Redman (in that tag team match) were rather tough. I know D-Mob was tough, but DMX, as well as Red & Meth were tough.
For Def Jam Fight for NY, I remember a lot of people thinking that Ice-T was tough, but I remember beating him on my first try when I first got the game. In my opinion, the toughest bosses were Crack (Fat Joe) and Xzibit. Sure, some other characters were tough, but those two took the cake for me.

That time you face the villain on the mountain top in Final Fantasy X.
Shao Khan in the various MK3 games. I've only beaten him by cheesing Liu Kang's low fireball.
Jeff Jarrett in TNA Impact. That final fight is completely broken, no matter how much you're beating him down, he generates a random special for the Stroke and you will not kick out of that shit.

Well, since I don't know how to make a introduction thread to this site, I guess I'll start off with some that really had a hard time facing off against. And I'll consider this my introduction.
As mentioned by everybody, a lot of fighting game bosses, especially SNK. The ones I really got bothered by the most include Geese in Fatal Fury (as well as his Nightmare Geese forms in Real Bout Fatal Fury Special & The King Of Fighters 2002: Unlimited Match), Any incarnation of Rugal Bernstein (THE boss that set the bar for years to come in terms of fighting game bosses), Alpha-152 in Dead Or Alive 4, Motaro in Mortal Kombat 3, Gill in Street Fighter III (especially in 3rd Strike), Azazel in Tekken 6 and...Ned Flanders in The Simpsons Wrestling. Or hell, way too many to list in MUGEN, for that matter!
For non-fighting games, the biggest example for me would go to Gate in MegaMan X6. I'll admit the fight had a good idea on paper (not to mention his bangin' boss theme), but the execution summed up the difficulty of that game (but X6 is still better than X7 by a country mile). At least when you defeat him, there isn't a Kaizo Trap of sorts.

I’m gonna go old school here
Conquests of Camelot from back in 1990. Fairly early in the game you have to win a joust against The Black Knight. The mechanics of the fight are pretty simple. The problem is that you simply can’t beat him. My roommate and I fought that guy about five hours a day every day for a week before we managed to beat him once.
Dragon Wars was from 1989. Midway through the game you have to fight a band of pirates and steal their ship. Cast all your defensive spells and load up your best weapons and prepare your best strategy….and it doesn’t matter because Long John Ugly and his band of pirates will still kick your ass over and over and over.

Luca Blight. Three separate fights with three separate groups of six then a one-on-one duel afterward.
The whole thing's extremely easy to botch if you don't know what's coming and plan accordingly.

Sephiroth - Kingdom Hearts 1 & 2 Karstaag - Skyrim Gill - Street Fighter III Weirdly, I remember in KH1 having an easier time with Sephiroth and Phantom than I did Frost Titan and Kurt Zisa.

Let's go old school with a few select Super NES bosses.
Scarface in Smash TV is egregiously hard. You have to take care of his entire shield and his face, and then he reveals an entire new set of shields and his bone face and he moves at a much faster clip with homing shots. It's also dependent on RNG if you can get lobbers to destroy his face. Even speedrunners tap out at that and stick to easy mode where it's just the first arena.
Kaiser Sigma in Mega Man X3 has a really small hitbox on him. It's aggravating if you don't have the Z-saber. What also doesn't help is the chase scene afterwards which makes it all the tougher.
Dracula in Castlevania Dracula X with all the pits and it feels more like a one hit death than a true fair boss fight.
