I was just about to mention Turtles in Time for the SNES, as that one came out at the same time as Hyperstone Heist. However, Hyperstone Heist came out only a few days before Turtles in Time, so I guess that might be accurate of Kevin Nash's first video game appearance being in that game. However, it's not like Konami used Nash's likeness for that character sprite.

TMNT III: The Manhattan Project has both Turtles in Time and Hyperstone Heist beat on this, as that game came out in February 1st 1992 (And on December 13th 1991 in Japan) and features Super Shredder as the final boss.
