Yeah, D'Lo's chest protector was my first thought. Shamrock injured him on screen and then he came back at KOTR with it and never looked back. It was Dan Severn. Not shamrock.

It was Dan Severn that injured D'Lo, with some submission hold, D'Lo then cost Dan Severn in KOTR I guess I'm looking at stuff that goes further into someone's career -- like if years after D'Lo stopped wearing the chest protector, the announcers would see him get put in some kind of stretch and remind us about that famous (but not actually real) pec injury, as if it were his biggest vulnerability. Chyna didn't really stick around long enough to have her kayfabe neck injury come up that way, but I feel like that could have ended up being one of these examples. Did Goldust occasionally stutter in later runs with WWE and other companies, or did it ever get referenced long after he stopped? If so, then that's what I mean. Luger's forearm, Michaels' back and some of the others listed here were real-life injuries.

Sting getting his feelings hurt after being accused of being in the NWO. Those emotional scars cut deep.

Matt Jackson has been selling a back injury on and off for about 3 or 4 years now. There may be an element of a legit injury there but I don’t think he’s ever missed extended time for it and it seems to always be the area opponents of The Young Bucks focus on in big matches. Nick Jackson can now sell eye injury for a while. Cause why not, just to troll everyone.

Joel Gertner wore a neckbrace for decades after taking Total Elimination.

I feel like Bob Orton's cast injury gets overplayed. I can't remember if he had it still at Wrestling Classic though I think he did. By Wrestlemania 2 and with the Boxing Bob gimmick he didn't have it and the LJN figure didn't have it. But I can see why it was so memorable.

Joel Gertner wore a neckbrace for decades after taking Total Elimination. This has seemingly been forgotten, but Gertner removed the neckbrace in July 1999 at ECW Arena, saying that he was finally healed. Seconds later, the Dudleys turned on him and gave him the 3-D. I believe that when ECW aired it on TV, they skipped Gertner's promo and just showed him getting attacked.

Goldust having the verbal tick and such after Evolution threw him into the electrical equipment.
Well, at least until Cody arrived on the scene anyway.

Isn't Malakai Black still pretty much selling that curb stomp into the steel steps from Rollins? First thought that came to my mind when seeing thread title

Roddy Piper has his hip injured by Adonis and Muraco and was still selling that in a feud with Hogan a dozen years later. To be fair didn't he break his hip in a motorcycle accident?

More an emotional "permanent" injury but...
Hulk Hogan never forgave Andre the Giant (kayfabe), did he? Even when he left the Heenan Family in 1990. Of course, Andre would never wrestle again on WWF TV or PPV, but he did appear for the Earthquake angle for instance. And him and Hogan never had that moment of reconciliation, aside from Hogan showing respect to him in (kayfabe) interviews after he died. And don't get me started on how he felt about "Andre's son" in WCW.
