When Michael Cole said Newcastle is a small mining community about a 3 hour drive from London.

I don't know if this counts, but ages ago I was watching European wrestling videos on the YouTubes, and they had English commentary on them by Orig Williams, who described Colonel Brody as "billed from South Africa, but hails from North Yorkshire" on air.
It makes no sense in that why would you just completely blow someone's gimmick on commentary like that?

During Sheamus vs. Dolph Ziggler at No Way Out when Ziggler was clearly the heel and the fans were chanting for them and you hear Lawler start to acknowledge the chants and then say "listen to them they're chanting...let's go Sheamus"

This probably won't count because it was part of a promo, but Jerry Lawler (commentator) telling Bad News Barrett "I have some bad news for you. Maybe next week, YOU won't be here!" Followed with a proud nod.

Lawler's reaction to this at WrestleMania X8 was priceless.
King: "What the heck is a Booger Red?" JR: "I'll tell you later!"
*Flair gets busted open later in the match*
King: "That's it, Ric Flair's Booger Red isn't he JR!?"

Gorilla's abuse of "literally" made for some insane statements. Guys were being knocked "literally into the middle of next week." They time-traveled, they were hit so hard! That caused me to cringe almost as much as when he'd say someone was "reeking in pain." The funniest part is that he would sometimes correctly say "reeling in pain," but other times would go with "reeking."
Unrelated, one of the biggest WTF exchanges in commentary history, from WrestleMania 3:
JESSE: "Tell me, Bob, is your mom and dad doin' the laundry after all this is over?" BOB UECKER: "Not anymore, but they, uh ... that's how I got interested in wrestling ..." JESSE: "I just wondered if maybe they [inaudible] ..." UECKER: "Yes, sir!"
And then Gorilla cuts them off to tee up a backstage interview.

Didn't Booker once randomly blurt out "Michael Cole I had a dream last night you DIED"? I think so, yeah, I know that isn't far off from the kind of stuff Corey Graves has said about Byron Saxton in whatever unexplained blood feud they seemed obsessed with putting over there, in what I don't think ever had an origin OR payoff.

I'm pretty sure Jesse just said that to mess with Gorilla and either Gorilla didn't or likely being a Frank Sinatra kind of guy and not being into modern Rock, Gorilla probably refused to acknowledge Axl. Gorilla seemed to have no idea who Axl was and when it seemed like he should have known, he tried to pretend. Even during the "wave" in the Jake/Ted match, Gorilla seemed totally oblivious to what Jesse was talking about. The celebrities at VI were probably more to Gorilla's liking-those born in the 30s and 40s. JESSE: "Tell me, Bob, is your mom and dad doin' the laundry after all this is over?" BOB UECKER: "Not anymore, but they, uh ... that's how I got interested in wrestling ..." JESSE: "I just wondered if maybe they [inaudible] ..." UECKER: "Yes, sir!" And then Gorilla cuts them off to tee up a backstage interview. What was the joke supposed to be? Or did it go over my head? Also, the Booger Red is something I don't think anyone understood. JR talked about it, I think with Conrad and even said "it went over like a fart in church." He was trying to create a new nickname for Taker and no one got it. It was based on football player (of course) Tommy Nobis who had red hair. As great as JR is and as much as he loves wrestling, I think his calling was really football.

That one time Jerry Lawler implied that more fans should like John Cena because he's really good at lifting heavy objects at the gym. Made even more comical because the King was over like rover in Memphis/the South and he always looked like he got all of his exercise mowing his lawn. lol Or by going to Burger King. (Remember from 1993 until the late 90s or 2000, fans used to chant Burger King at him? Jerry Lawler as late as the RR 2000 beauty contest where he talked up Barbara Bush (not the former First Lady but a wrestler) he said that people call him Burger King.)

Didn't Booker once randomly blurt out "Michael Cole I had a dream last night you DIED"? This made me laugh hard as hell just for how out of nowhere and odd it was. That and Booker claiming he just had to go to the Bathroom the week after the entire roster walked out on Triple H

Just re-watched WrestleMania 25 and relived this "gem":
King: "I wonder if Rey Mysterio is paying tribute to Heath Ledger's Joker?" JR: "Rey Mysterio's career is alive and well, King."
Like I still have no idea what purpose that comment had other than to take a dig at a man's recent death, and it wasn't even from a heel commentator.

Not exactly commentary, but it was a line that made no sense
Jim Cornette, relayed a saying that Chief Jay Strongbow would make about a bad match: "They could have had that match in an Iron Lung"
Bruce Pritchard repeated it once in a meeting and Vince turned around and said "WTF is that even supposed to mean?"

Jesse's Axl Rose comments from Wrestlemania VI. I think he said Robert Goulet reminded him of Axl. They are two total opposites. Plus Gorilla had no idea who Axl was. And it seemed to make Gorilla look foolish he didn't know so he tried very badly to pretend he did. There was only like a 10-12 age year difference between the two but it felt like 20-30 years. Coliseum cut out most of what Jesse said (because they cut out Goulet) so it seemed even more awkward on the VHS. I liked Jesse and Gorilla better but if there was a pop culture reference, Gorilla did better with Bobby. I'm pretty sure Jesse just said that to mess with Gorilla and either Gorilla didn't or likely being a Frank Sinatra kind of guy and not being into modern Rock, Gorilla probably refused to acknowledge Axl. Granted Gorilla had his fair share of irrational comments. I remember watching an Owen Hart match where he talks about extraneous circumstances that allowed Owen to win. Ted DiBiase on commentary likely actually puzzled asks him, "Like what?" and Gorilla just goes, "You know what I mean". Gorilla likely forgot Owen won clean and realized it during the match. As a broadcaster myself, I can tell you sometimes you say something that makes sense in your head but to no one else. I love Gorilla, but he seemed so lost a lot of the times that he did commentary.

Uecker once mentioned in a promo with Gene Okerlund that his parents were friends with some pro wrestlers when he was growing up, and they would stay at the Ueckers' home sometimes when on the road.
