I'm sure there are some guys he dislikes, but his rants about Hogan are a work as I saw a video on youtube of both of them together recently and they were very cordial towards one another.
I also saw his other shoot that he did in 2000 with RF and he was a lot calmer and more serious. He didn't go on a tirade every few seconds.

I think Sheik is the type that lives his character and doesn't like breaking kayfabe.
No doubt that he is nuts and completely insane, but a lot of what he says is likely for the camera.

Wasn't there an interview with someone that did a shoot with him who before hand Sheik asked him "Do you want crazy Sheik or do you want me to be normal?"

Probably a mix of both due to his age and years of drug abuse. Plus, he may need the money and this is the only way he knows how to earn money and attention. On some occassions, I see him as the old guy who really needs attention (think of Abraham Simpson complaining about how evil Krusty is).
