I'm inclined to think Super Mario Bros. 3, but I only remember making it to the 7th world (the pipe world) one time when I was 4 or so. Other than that, I can't remember for the life of me.

sonic the hedghog or golden axe on the megadrive/genesis.
commodore 64's just didnt have the ability to play a game the whole way through without crashing/melting.

Super Mario Brothers.
Almost made it all the way through with firepower all through the 8th level, but got knocked down to small as i got to Kroupa.

I don't know for sure, but the first game I actually remember beating was Mega Man 3.

The first game that I ever beat was Squish'em Sam on the Colecovision. Ten internets to anyone who even has heard of that game.
You got to remember that most of the games from my early youth never ended, they just cycled through again at a higher difficulty until it could not increase, at which point it merely cycled. Hell, even the Smurf game on Colecovision did that, so that once you rescue Smurfette (and hopefully get your well deserved reward off-screen), you have to brave the Ledges of Doom, the Grass Blades of Steel, the Fences of Peril, and the Smurf-hating ravens and bats to go rescue her ass again.

Resident Evil for the PSX..except I couldn't hold my nerve when trying to rocket launch the Tyrant on the roof so my friend did it for me. I was eight at the time. lol
Also I was the first one out of me and my brother to get the Jill ending when you kill Barry!
...ok he gets garroted by lab monsters, you don't kill him.

I am notoriously bad at 8-bit and 16-bit games so I don't think I ever completed any of the games I played on the NES and SNES as a kid... oh, wait, yes I did, I actually saw all the endings of Killer Instinct on the SNES. I guess that would be the first, then.
I did feel pretty good when I finally beat Mega Man II last year, though. ;D
