My favorite one was the Rey/Eddie ladder match from Summerslam 05. Apparently, Vickie was supposed to run down and distract Eddie off the ladder or title, but didnt, so Rey had to yank him down (nearly blowing out Eddie's knee in the process of landing).
What elevated that to legendary botched cue status was Eddie on the mat screaming to a ref loud enough for the audience at home to hear perfectly clear, "*** DAMMIT! ************!!!! WHERE THE **** WAS VICKIE?! *** DAMMIT!!!!"
Razor Ramon forgets to move when he's held by Diesel for a Sweet Chin Music from HBK. HBK stopped his leg a little for Razor to move (Summerslam 1994, I think)

I don't know how it ranks among the others. But fresh in the mind was the beginning of what turned out to be an excellent segment.
When Orton went to punt Piper, he just stopped and stomped on him. Kofi's music hit not a second later.
Not sure if that's what inspired this thread or not.

Bossman nearly ruining Shane McMahon's heel turn at Survivor Series 1998, by not coming to hit Austin, forcing Austin to stand there like an idiot as everyone at ringside had to come up with a new finish on the fly.

When Lesnar had Heyman up for the F5 during a cage match and had to walk around with him forever while Angle tried to get inside, and then finally just climbed over.
Also, a similar cage goof involving Mark Henry and Tista.
Edit: Also, Rico yelling at Jeff when he forgot a spot on PPV during one of his more drugged out periods.

Jeff Hardy watching the pinfall in that scramble match from a couple of years ago and jumping to pin someone else, it made no sense whatsoever because he was already the champion at that point.
Also when the bell went before the 3 count during Bobby Lashley and Umaga at Wrestlemania.

When HBK put Bret Hart in the sharpshooter you can CLEARLY see Hart tap out like a little girl. Yet because of the botched camera angle, all the Hart Fans will forever complain that HBK and Vince screwed Bret.

I didn't see that ppv. please explain? Papa Shango was supposed to do a run-in during the Hogan vs. Sid main event. I'm guessing he was supposed to interrupt the ref's count after Hogan hit the legdrop, but because Shango missed his cue, Sid had to kick out of the legdrop, which (back then, anyway) was a massively big deal. Anyway, Harvey Whippleman (Sid's manager at the time) gets on the apron and tries to save the finish, and the ref ends up calling for the bell on Whippleman's "interference" (which basically amounted to Hogan bringing him into the ring by force). Papa Shango comes down during the commotion and the beatdown commences, Warrior's music hits, and the rest was history. But that's possibly the only time I can imagine a WM main event having its finish blown like that (unless you count Michael Cole letting it slip during a plug for the aftershow that Austin was going to win the title at WM15), although if you watch it, it's really not that bad. The hot crowd sort of covered it up. At least as a kid, nothing was ruined for me. That said, the spurious nature of that DQ stood out to me even then. EDIT: Ah, here we are:

I didn't see that ppv. please explain? The main event was Sid Justice vs. Hulk Hogan. Papa Shango was supposed to come out and cause a disqualification. Hogan hit the big boot and slammed Sid. Then he dropped the leg and covered him. Sid kicked out on 2. Confusion all around so Harvey Wippleman jumps in and causes the dq. After the dq, Papa Shango comes out to attack Hogan. It was revealed that Shango was supposed to come out on the leg drop to cause the dq.

The RKNOOOOOOO on Chris Jericho. Battle Royale on Raw. Jericho had someone in the Walls. Orton tried to do an RKO off the ropes, and either Jericho didn't react, or Orton totally missed it. I'm not sure. But Orton ran back to the ropes and did it right the second time, like the trooper he is.

Granted, that did help Mark Henry a TINY bit, but 2 minutes were devoted to Bertista (my nickname for him over in CBR Forums) just looking at Henry trying to find out why the cage was double-bolted. Bad move. Isn't Jamal yelling at Rico to get the spot done, as well? I mean, it's obvious that this was the point where the Dudley Boyz were to be eliminated, so Jamal had to spend the time just punching Bubba Ray while Rico's bitching out Jeff to set up the spot. Boy, the crowd sure caught on fast. I don't know any such cases that have happened lately outside of the Nuggets/Lakers match, but that was more of a "botched move" than a bad call or goof.
