I've been to Arbys when they ran out of beef, & Subway when they ran out of bread. I once ordered two cheeseburgers from McDonalds. Get home, and they put ketchup, mustard, cheese, and onions on the bun, but no burger. I go back, and the dumbass made the entire line of burgers that way. My friend had that happen at McDonald's one day with a Chicken Bacon sandwich. He opened it up to find bacon and all the trimmings but no chicken patty. I caught him at Dunkin Donuts as he worked there and he showed me the sandwich. I told him to call because the owner was cool as hell, but he just shoved an Omwich patty in it and ate it. My grandmother once ordered like 20 cheeseburgers and 2 hamburgers from that same McDonald's. She got the numbers flipped and called out. My grandfather drove out and got a new order ontop of the old order as well as a $20 gift cert. Granted, it was the owner himself who was working the line that day. My bro and I ended up going around the neighborhood, giving out free hamburgers. My friend had the same thing happen only with happy meal toys getting reversed. His gram ended up getting the same stuff, only they owner delivered it out to them as it was a family reunion (and this was at least a 20-30 minute drive.) Every so often at breakfast, that McDonald's wouldn't have OJ when my mom and I would stop. They'd always give me a super-sized orange Hi-C instead. That McDonald's was pretty good though, the overwhelming majority of the time. For a good while after the Hardee's in town went out of business, it was just them and Wendy's. Wendy's was across the street but had an awful drive through setup and was still geared to more of a eat in restaurant at that point. Now I live in a town with just a BK. They've messed up only a couple times, usually just not hearing part of a long order. Once or twice they didn't put a $1 sandwich or something in, but I never bothered calling or anything.

Went to McDonalds for breakfast yesterday morning. I ordered two Sausage, Egg, and Cheese biscuits and an order of hashbrowns (you now get two for a dollar). After I get my food in the drive through I pull up and check the bag. Look inside and all is good so I head home. It's right about 11:00 when I get there, sit down to eat and find they put McGriddles in the wrong wrappers. I was pissed, got in my car, and called them on the way back. They gave me all new food even though they were serving lunch. It worked out though, because I could tell my food was fresh and the McGriddles were old.

I walked into an El Pollo Loco at 5pm one day and was told that they ran out of chicken. If I wanted to wait they would have some more ready in about 45 minutes.
I have been to several sandwich shops that have run out of bread. Usually when it is close to closing time.
My regular McDonalds ran out of hashbrowns one morning. They put a coupon for a free breakfast sandwich in the bag instead.

I orders two fish sandwiches from LJS and the cashier asked me to reconsider because they didn't have any lettuce.
