I've always wondered how they do things like this, i don't know if anyone on here knows. I'm assuming he was in the curtain part at the bottom but still, seems difficult to pull off.

It was obvious there was a fake bottom on the casket and he was under the shroud.

They made sure to keep the casket out of frame after Kane closed it. The first chokeslam was likely the signal for Taker to start moving up into the casket proper, with it not being in frame, any movement of a gigantic man moving around in there isn't noticed. Kane gives Rey another chokeslam and a Tombstone, basically counting the beats as a way of letting Taker know how much time he has left to be in place, and have it look like the bottom of the casket is still there. Kane takes his sweet time dragging Rey over, and Taker has had ample time for the spot to be set up.

Still, though...it seems kind of hard to go from underneath a casket to up inside a casket without:
A) Tipping the whole mess over, or B) Actually getting out, opening up the top and climbing in.
I wouldn't think Taker would be up to that kind of agility with his messed up legs. Maybe that's why he was coughing so much. Dude was winded.

There's no doubt in my mind that it had been rehearsed until they got it down right, and Taker's a pro.

He was coughing because he was selling the fact he just got out of a coma.
