It probably depends on the show, and even then, if it's Raw, it's probably different for one of the super episodes, in which case it might even be Vince.

I'd imagine creative comes up with ideas, the bookers decide what they will accept, ignore, or put them on different wrestlers, and Vince and the top brass ultimately gives the final seal of approval.

Side note: who IS the booker for Monday Night RAW, exactly? Because, now that I think about it, I've heard everyone from Ace, to Stephanie, to Vince McMahon himself blamed for the current condition of RAW, and Smackdown for that matter. Rarely does anyone actually blame the bookers for WWE's TV product (I think one poster here has consistently called out Smackdown booker Ed Koskey).

Brian Gerwitz is still the main writer of RAW, isn't he? I wanna know who writes NXT personally. Nah, he got promoted to an executive role along with PS Hayes. I believe Dave Kapoor, the guy that used to manage Khali, has rRaw and, Ed Koskey has Smackdown. That said, Vince still decides who gets what push and has final say.
