I'll second Shadow Hearts, but only the first one as far as disturbing goes. The second one is a better game but ups the comedy in place of horror. The third one was kind of disappointing.
Most of the Shin Megami Tensei series as well, especially Nocturne and the Digital Devil Sagas.

I've never played this game, but I have read about it. Look up "Ethnic Cleansing" and if that doesn't disturb you, then I don't know what will.

My buddy came back from Afghanistan with a game called BONETOWN (don't google it from work). It's highly disturbing in many, many ways. Gah! That's disturbed in a completely different! What's so disturbing about the game? XD

I've never played this game, but I have read about it. Look up "Ethnic Cleansing" and if that doesn't disturb you, then I don't know what will. It hardly counts as a game. The thing was presumably made on a poorly-used game maker, it's a glitchy mess, the 3D models are laughable, the AI is non-existant and some parts just plain don't work. I tried it out of curiosity and all it does is prove that neo-nazis are useless, underachieving assholes who prefer to blame people they never met for their problems rather than actually do anything about it. It really doesn't deserves the publicity it got, it's just not that important, just like neo-nazis.

It will seem slightly primitive now, but I Have No Mouth and I Must Scream is pretty unsettling in its own way. I only discovered that game long after it came out (I hadn't heard of the short story beforehand either), but man, that game is underrated. Way ahead of its time, in terms of storytelling and using games thematically. Before morality-based choices became mainstream in present-day great games like Bioshock, there was I Have No Mouth and I Must Scream.
