They do specify it sometimes as "No DQ, no countout" matches. Which means its possible to have a countout in a regular no dq match. Not much point though.

No Disqualification match[edit] A No Disqualification match, also known as a No Holds Barred match,[30] or sometimes as an Anything Goes match or Raven's Rules match, is a match where neither wrestler can be disqualified, allowing for weapons and outside interference.[31] The key differences between a No Disqualification match and a standard hardcore match are that falls must be made in the ring, there is less emphasis on the use of weapons, and often the Count-Out rule is still in effect for No Disqualification matches.[32] However, in a No Holds Barred match, the Count-Out rule is not observed, with a greater emphasis on weapons as opposed to a No Disqualification match. A match which does not observe disqualifications or count-outs, where pinfalls must take place in the ring, can also be known as an "Unsanctioned match", or, "Street Fight".
So No DQ =/= No Holds Barred/Street Fight.

Well I guess now when we ask "What's the difference between a no dq match, street fight, no holds barred" etc matches, we can say that a no dq match can still be won by countout. Unless they say specifically that it's no countout anyway.
Of course that doesn't make this look any less goofy. I can't actually recall any other no dq matches won by countout. It's so consistently "no dq no countout" that it's just kinda assumed.
I don't remember people brawling outside the ring in no dq matches with the ref there counting either, it's not like they clarify "no countout" in most of these no dq matches. We've heard "no disqualification" with no mention of countouts hundreds of times without the ref counting out anything. He usually just sorta looks on disapprovingly and says "don't do that!" when someone's about to be powerbombed through a flaming table of puppies.
As usual with wrestling, it's whatever they feel like doing. Most no dq matches they don't do a 5 count for ropebreaks either, but they do once in a while. Which makes you wonder what exactly happens if the wrestler just ignores the 5 count. I guess it'd be one of those deals where the ref physically breaks the hold rather than calls for the bell, which you do see every once in a while.

My biggest issue is, logically, the match this week should have been no countout, based on the result of last week's match, do a DQ finish this week, then a no DQ next week, with Ryback standing tall before getting destroyed at the PPV.

That was the deadest crowd for a main event I've ever heard. There was hardly a peep when Cena went for the 5 knuckle shuffle.
