Now that that's out of the way...honestly, have him face Cena at WrestleMania, say beforehand that win or lose, this will be the last contest that The Undertaker will fight. Keep the streak.
Then next year, Hall of Fame.

Beats Cena via Hell's Gate within the next two Wrestlemanias. The next night he cuts a half-Deadman, half-real life Mark Calloway promo for Raw's closing segment. The roster comes out dressed as druids and applaud him. Taker does his signature pose, then does a vanishing act to end Raw. His next live appearance is his HOF induction, of course.

Another idea I had a few years ago, that wouldn't work as well now due to Undertaker's limited appearances, would be him training a successor.
Basically, start up a tag team of The Undertaker and this new guy (I was thinking someone with a similar build and such to Undertaker...before he became Luke Harper I was thinking Brodie Lee but whatever). Half a year of teaming, winning the tag titles together, the guy getting slowly better and Undertaker even allowing him to use the Tombstone.
Then one show, Undertaker comes to the ring, with the new guy. Undertaker informs him the time has come for him to move on to another path, and the new guy will carry on the spirit and power of The Undertaker. The lights go out, when they come back on The Undertaker is gone, the new guy is in Undertaker attire save for the hat. The hat is laying at the new guy's feet. The new guy reaches down, picks up the hat, and puts it on...the gong going off as he does. A new Undertaker (though not necessarily with the name Undertaker) is born.

Have him and Kane face off at Mania one more time, but let it be known that for both men it's their final match. Taker wins, him and Kane show their gratitude for one another and then the lights go out. The lights come back on and in the middle of the ring lies the urn and Kane's mask.

I know it's never going to happen but I want him to do one last run as The American Bad Ass.
Not the douchy short haired generic tall guy babyface biker or heel bully biker he did in later years but the original incarnation of that gimmick where he came down on his bike in his trenchcoat and shades chasing people round the ring on his bike and booting the shit out of and chokeslamming everyone and being an all round bad ass. (man they really f***ed the biker gimmick up in later years)

I think some sort of dark/creepy/gothic gimmicked wrestler should be the one to retire him.
Bray Wyatt would be a good choice. Likewise, depending on how over they get when called up, The Ascension.

The Undertaker's last match should be a Buried Alive Match, which he loses. After the match an accordion player and a singer come out and stand on The Undertaker's grave and perform this song:
Lights go out and come back on and The Undertaker's hat and duster jacket appear. The singer picks both up, folds up the duster jacket and places it in the hat and leaves (only to appear again if you want to put someone over as being the next Undertaker-like character).

Didn't Triple H mention that in promos prior to their Hell in a Cell rematch at Wrestlemania 28?! "It's best for business that you stay in the company and compete year after year, I won't be in a rematch as I'll destroy you." *Undertaker cuts hair* "This keeps happening, I destroy my image and my character if you don't face me!" "Oh crap, okay then, game on." Has Triple H been considering this as a character arc for that long?
