When they showed his hip surgery...UNCENSORED...on the WWF syndie programs? Eventually there was a pixellated version, but I remember it mostly being seen unaltered.
My question is...given the target audience of the WWF at the time, why? And how did they get away with it?

I don't think I saw it when it first aired but saw it on Youtube a few months ago and was completely shocked at how graphic it was.

I had forgotten about that, I want to say that I saw similar pictures in a wrestling magazine as well. Anyone have the youtube link so that I can confirm?

Not really, if you stop and think about it. During the Olympics! the TV. networks broadcasting the games will do documentary stories on competitors who have interesting back stories in order to put a personality to the name and face. This was a cool way of doing the same thing in the context of a wrestling show, which, IMHO tried to give pro wrestling (specifically the WWF) a sport-like legitimacy.

I remember the vignettes that did for Superstar. They showed his surgery and road to rehabilitation. I especially remember the one of him walking the mountain. I forget the heels who attacked Graham after he made his in ring return but it was enough to put him back on the sidelines and he eventually left the promotion. Back then, it could be flagged simply as education for the kids, or something to that effect. It wasn't until the mid 90's that education programming for kids block became mandatory. Beforehand, a vast majority of kids shows where just toy commercials and mindless fun. In order for a tv network to air their usual kid programming, they had to provide educational counter programming. Usually they aired these shows in the morning and stuff like Power Rangers in the afternoon.

God, he was such a bad commentator, unless your ex huff fan of a guy over and over saying "hey man, look at this body... Look at the muscles"

This along with Ken Patera was a great comeback story that unfortunately didn't pan out. They need more stories like this, even if it were kayfabe like Steamboat coming back for Savage.
As someone mentioned Ron Bass carving up Beefcake's head-did they ever show that uncensored?

I think the first time it happened - meaning you were actually watching that episode of Superstars - then yes, it did air uncensored. Since then, it aired with that crazy red X on the screen. (If memory serves me correctly, they tried using black and white and it really didn't work out as well. Then another time, they would cut out the picture, but we heard the audio of the assault.) Crap, I was more freaked out by the red X than the blood.

The X made a comeback, on the other hand, the first time they showed Macho Man being bit by the cobra. Then after that it was always seen uncensored. I still say 9 times out of 10, the Superstar surgery footage was seen uncensored. I just remember it being pixellated once or twice. They didn't need to show it uncensored to get people behind him, IMO, and that graphic footage wasn't really meant for the audience at the time, so I still wonder if there was some double agenda there. When WWE went PG, there was the brief time they went black and white on classics on demand whenever blood was shown ANYTIME. I think they did that for promo packages even in the Attitude Era. Now they just rate bloody shows TV-14. Funny "censoring"...back when NWA Wildside had a TV show on Urban America, when someone was shedding blood (usually Iceberg) they'd put a black screen with Q*bert-style cursing gibberish. Going to the WWF again, there was how they censored Damian's "death" by cutting to Sean Mooney making shocked faces. And let us not forget the random crowd shots (usually the same one with a homeless guy who dressed like Where's Waldo) when someone used any kind of weapon in the GWF.
