My guess is Extreme Rules, with 5/6 on the card. Anything higher than that?

WrestleMania X8:
1. RVD d. William Regal for the IC title. 2. Spike Dudley d. Maven for the Hardcore title. 3. The Hurricane d. Spike Dudley for the Hardcore title. 4. Mighty Molly d. The Hurricane for the Hardcore title. 5. Christian d. Mighty Molly for the Hardcore title. 6. Maven d. Christian for the Hardcore title. 7. HHH d. Chris Jericho for the Undisputed WWE title.
The Hardcore Battle Royal from WM2000 probably trumps it easily.

How about the J-Crown? There were 8 titles on the line, and a total of 12 title changes through the show:
- The Great Sasuke had the IWGP Jr. Heavyweight title going in and kept it (0 changes). - Masayoshi Motegi had the NWA World Junior Heavyweight Championship going in, lost it in the first round to Sasuke (1 change). - Gran Hamada had the WWA World Junior Light Heavyweight Championship going in, lost it to El Samurai in the first round, who lost it to Sasuke in the second (2 changes). - El Samurai had the WWF Light Heavyweight Championship going in, lost it to Sasuke in the second round (1 change). - Ultimo Dragon held the WAR International Junior Heavyweight Championship going in, lost it in the final round to Sasuke (1 change). - Jushin Liger had the British Commonwealth Junior Heavyweight Championship going in, lost it to Ultimo Dragon in the first round, who lost it to Sasuke in the finals (2 changes). - Shinjiro Otani held the UWA World Junior Light Heavyweight Championship going in, lost it to Ultimo Dragon in the second round, who lost it to Sasuke in the finsls (2 changes). - Negro Casas had the NWA World Welterweight Championship going in, lost it to Otani in the first round, then went to Ultimo Dragon, then to Sasuke (3 changes).

The J-Crown tournament took place over four days, not one day. But I think the only recognized title changes during the tournament were the finals, so that would mean 7 title changes in one match. That could very well be the correct answer as far as serious championship go.
According to wikipedia, DDT's Ironman Heavymetalweight Championship changed hands 62 times on one show.

There was a BIG stretch of title changes between two guys that happened via rock-paper-scissors.
Some research tells me that Shinobu and Yuko Miyamoto traded the title a whoopin 304 times between them! And then they did the 62 times, again between them.
