I'd agree with the Faarooq Asaad-Ahmed Johnson feud, but I tend to think the injuries to each other made the feud a lot more "personal".
I'd like to think the work was laid down for a possible "Wildman" Marc Mero vs. Rocky Maivia match at WM 13. On an episode of Shotgun Saturday Night, Marc gets beat down in a match against the Sultan (I think), then he and his "handlers" surround Sable. Rocky makes the save and chases them off. Marc takes exception to it and drags Sable away. Marc blows out his knee soon after and is off TV until the tail-end of the year. That's when they restarted the "Marc is jealous of Sable's popularity", and had him teaming with Goldust. Then again, he wasn't gonna feud with Rocky, who had just skyrocketed past Marc Mero level and was busy planning a coup of the Nation.

There was the Andre-Earthquake angle in 1991, although that's sort of a backwards example since Andre already had the physical problems when the angle was started. I've always wondered if they expected Andre to recover enough eventually to have a match against Quake. Walking around on crutches with the Bushwhackers was such a lousy way for him to go out that I have trouble believing it was designed to end that way.

Someone beat me to it, but the Power Trip. That quad injury...
Dolph Ziggler's concussion really ruined his World Heavyweight Championship reign.

One that may get overlooked: Goldberg vs the NWO Most involved say the plan was for Goldberg to tear through the entire newly formed NWO and get the title back, but he put his arm through the windshield of the limo and missed about 5 months. I've always wondered about that plan, but I've also wondered how things might've been different had they gotten a chance to go through with it. Not to say it would have prevented WCW's eventual demise, but going from the Fingerpoke to Goldberg being off TV for awhile was a huge blow during that pivotal stretch in early 1999. Golberg hurt his knee in 99. Was gone until late Summer. Cameback the NWO was gone and then cameback together at the end of the year and then he got hurt with the limo in 00.

The story I always heard was that it was supposed to lead to Hogan and Andre making amends and then have some kind of program against the Natural Disasters. I guess they were hoping Andre would recover enough to atleast work some tag matches with Hogan carrying the load.

Not as soon as it started, but I'm guessing Magnum TA's car accident led the NWA to go a different direction with a contender.

Didn't Morrison get hurt right after his feud with Truth started? That should have been all the better, with Morrison returning months later to exact his revenge on Truth. Instead, he comes back too early and gets beat several times to no fanfare, and couldn't even get their a feud a one-on-one match at a Summerslam where only three matches were announced a week before the show despite being in the works the longest of all the active storylines. He finally gets his Falls Count Anywhere match the night after and bets Truth, and then immediately gets a losing streak angle for his troubles, and that wouldn't be the last of his indignities in those last few months. WHAT HAPPENED THERE?

The planned Steve Austin/Steve Williams feud. Dr. Death was supposed to plough through the competition and use the Brawl For All as a springboard to establish him as a main event talent... They never saw Bart Gunn coming.

except the limo angle was a year later That NWO version formed in December 1999 after Starrcade. At the end of the month is when Goldberg got hurt on the limo spot. He didn't come back until May of 2000. Gotta love Kevin Nash and how he conveniently forgets his timelines on shoot interviews when talking about this

Most people forget about it, but Evolution's original formation was sabotaged. The stable formed in early 2003, but Orton and Batista both got injured in the same tag match on a house show immediately afterwards, leaving Triple H and Flair alone until their staggered returns.

They shot one angle on Raw with Waylon Mercy watching an Undertaker match. I believe that was the last time we ever saw ol' Waylon. Ha @ the idea of Mercy being so scarred of what he saw in the match that he decided to leave wrestling all together
