I've heard that Dolph Ziggler was supposed to win the IC title in his 2009 feud with Rey Mysterio but Rey-Rey put the kibosh on it because he didn't feel that Ziggler was ready. Welp, add that as another reason I detest that little cockroach.

Not for a title (but roughly related as it had to do with KOTR placement), Austin going home instead of losing to Lesnar. Actually, Austin was infamous in 1999 for refusing to do jobs. There were the HHH and JJ examples, but I think Billy Gunn was another? In retrospect, I would guess we'd not have a problem with his refusal with any of those...
Bulldogs wanting to only lose the WWF tags to the Hart Foundation (IIRC, they recommended they lose them to Sheik and Nikolai)
If the urban legends are to be believed, Ric Flair vetoing Dusty Rhodes' idea for him to lose to Rick Steiner.
Pretty much any time HBK had a title from 1998 and back? (others were named of course, just thought of the one where Bulldog was to retain the Euro title at One Night Only, but Shawn wanted to win that night?)

^ Austin refused to work feuds with Jarrett and Billy Gunn. Jobbing to either man wasn't on the cards but apparently Steve saw working with both men as a waste of his time.

Wasn't Foley added to the Summerslam '99 main event because Austin didn't want to put over HHH? I"ve heard that Foley won that match because of that or Jessie Ventura didn't want to the raise the hand of a heel. Not sure which is true. Wouldnt have been difficult for Triple H to win then have Ventura raise Austin's hand after a Stunner.

Would Austin and HBK refusing to do singles programs with Owen count? After Summerslam 97, could you really blame Austin for not wanting to work with Owen again? "Hey, ya never even called me once after breaking my neck but sure, I'll let you make some big money with me."

Lou Thesz would not drop the NWA World title to Verne Gagne. Not because Verne wasn't over (he was hugely popular) or because he wasn't a skilled wrestler (he was). Politics behind the scenes scared the Muchnick camp into wanting to keep the belt "in house". (Muchnick was afraid Fred Kohler of Chicago was going to take over Verne's bookings for the NWA title, thus shutting out some of the territories on the NWA scene, including Muchnick himself.) Lou was also a bit upset that for years, Verne Gagne holding a newly-created "United States title" on the Chicago wrestling show would sometimes be treated as a bigger deal than Thesz' belt. Even after changing it to a "United States TV title" and having a proclamation made that Gagne's belt is a regional title - much like the British Empire title or anything similar - but Lou is the recognized World Champion, Thesz wasn't happy. Thesz did put over Dick Hutton, a former NCAA Champion, but also a colorless guy fans didn't really care to see with the belt, because he fit all of Lou's ideals. (Dick was basically a gatekeeper for when Lou came back from taking a break.) Thus, Verne set the wheels in motion to just branch out and create his own area by buying out the Stechers in Minneapolis. (He still recognized the NWA Champion, just not as the top guy in their territory.)
Same deal was pretty much the foundation of Capitol Wrestling breaking away from the NWA in 1963. The group didn't want Thesz to regain the NWA title from Buddy Rogers because Lou just did not draw in the Northeast territory. So after Buddy lost, they came up with the reason that "Buddy only lost in one fall, all title matches are supposed to be 2-of-3 falls. Therefore, we recognize Buddy is still the champion." When told they must recognize Thesz, the group pretty much gave the NWA the metaphorical finger and left. They weren't the only ones that left that year, Western Texas/Amarillo also began recognizing their own World champion (Gene Kiniski) because Thesz never stopped in their area. Funny thing about Amarillo leaving the NWA, their promoter Doc Sarpolis was the NWA President at the time. And remained President despite their exit from the Alliance.

If Austin refused to job to HHH at SS 99...so what? It's not like HHH was remotely top guy over at that point or had any business beating Austin. Just weeks before he lost his #1 contender status to CHYNA on Raw for crying out loud. There's a lot of revisionist HHH history really. The dude wasn't over at all in 1999 as a heel. He struggled mightily to get over after leaving DX and it wasn't until Foley nearly killed himself to put HHH over that HHH got over as a main event heel.
Speaking of which, Foley is such an unsung hero. The guy demystified The Undertaker in 1996 and made him a more human character, made HHH look like something more than a lame Connecticut blue blood in 1997, got Kane over in his initial push, made Austin look great during his initial title run, made Rock into a semi legit world champ, and finally made HHH a main eventer. All while rising in the ranks of popularity himself. Think about that for a minute. Foley made Austin, Rock, and HHH all look like gold in 1998-2000.

Did Backlund refuse to drop the title to Hogan? I think I remember reading that somewhere. I don't think it was that he refused to drop it, he just didn't want to do it as a heel, as per Vince Jr.'s wishes. The deal with Backlund is actually that he refused to drop the belt to anyone who didn't have amateur credentials; hence why they chose a past-his-prime Sheik.

Sabu left the original ECW once and for all after he refused to put over Super Crazy. He also left the WWE version of ECW a few years later after refusing to put over Kevin Thorne. It should be noted in both cases he already had long standing issues with both promotions and would probably be out the door sooner or later anyways.
Ahmed Johnson was supposedly fired from the WWF after refusing to put over Kurrgan.
While I don't think it was ever put to the test, The Ultimate Warrior supposedly had a "no jobs" clause in his contract when he re-signed with the WWF in 1996.
During the original ECW invasion of the WWF back in 1997, the whole angle supposedly fell apart after Rob Van Dam refused to lose a match to a pre-Road Dogg Jesse Jammes via a screwy finish on a Shotgun Saturday Night taping.
At Road Wild 1997 the Steiners were supposed to win the tag titles from the Outsiders until the Outsiders threw a fit right before the match and got everything changed to a DQ finish. The Steiners winning the tag titles was supposed to balance out Luger losing the World Title back to Hogan in the same night, but instead WCW went back to the "we can never make the nWo look vulnerable under any circumstance" line of thought that ultimately killed peoples interest in the angle.

Ever wondered why Austin started wearing two knee braces at that event? He messed up the other knee putting all his weight on it when Chyna low-blowed him during his corner toast celebration a few weeks prior. Rather then take Austin, the champion, out of the main event of SummerSlam, they added Foley to take the slack off of Austin.
