You can delete 3 championship reigns and also add 3 reigns. Here's mine.
Delete - Miz's 2013 IC reign (he won it at Mania, held it for 1 day before Barrett gained it again, useless) - Del Rio's late 2011 WWE title reigns (Punk should have kept the belt the entire time and never lost it) - Triple H's Reign of Terror (Honestly, I don't care who but one of them shouldn't have happened)
Add - Booker T WHC 03 reign (He should have beaten Triple H, no questions) - Ziggler 2013 WHC reign (He should have beaten Del Rio for the belt legit, held it for a few months before losing, he looked like a bitch never getting it back) - Primetime Players 2012 tag reign (They should have gained the titles sometime in 2012 before Team Hell No, they were on fire)

Miz's WWE title reign
Orton's 2013 WWE title cash in (thus giving Bryan an actual title at least 60 days and then they could proceed with the stupid bullshit they're doing now without me being annoyed by hotshotting the title around)
Mysterio's WWE title "reign" (if you can't hold the title for 24 hours, it shouldn't even f***ing count)
I'd probably add on Andre's, Yoko's, as well as Orton, Cena and Trips' recent under a day wins a well, but I'll abide by the "3" rule.
Rude beats Warrior for the WWF title in 1990 or 1991 (if Rude stuck around, I would have gone to him to be the bridge from Warrior to Hogan, since I hated Iraqi Slaughter and it still remains as one of the worst main event angles I remember from my childhood)
Bam Bam Bigelow wins the WWF in 1995 (always pissed me off what WWF did to poor Bam Bam, the guy goes out there and busted his ass to make f***in' LT look like a million bucks and they don't follow up with him in any way)
Christian wins the WWE title over Cena in 2005 (I don't even care if I'm the only one who would have been happy about this, or if it did bad business or whatever because I believe Christian was at the top of his game, and could have done the ultimate opportunist gimmick better than Edge was ever able to)

Chris Jericho's first WWF title reign - If they were just going to pull the rug out from under him for a while, there was no point in him going over Triple H in the first place. Just squash him and move on to someone else, please.
Hulk Hogan's WrestleMania 9 title reign - There was no point to Hogan charging out to win the belt in a matter of seconds, since he would turn right around and drop it right back to Yoko at King of the Ring. Skip the intended feel good moment and keep the strap on Yoko from the start.
CM Punk's 400+ title reign - Started out decent enough but by the halfway point, nobody was going for the heel turn and everyone just kept bitching about how he was just keeping the belt warm for the Rock. Probably part of Punk's beef about coming so close but still not main eventing Mania. Cut the reign in half and put the belt on...

Alberto Del Rio - He could have played up his character's strengths and still gotten beaten half to death by Ryback without dragging the Shield or Maddox into it and then jobbed the belt to the Rock without a problem. That way Rock's title run is met with unanimous elation across the board.
Booker T - He should have beaten Hunter at Mania and run with the gold for at least six months.
Mister Perfect - I would have loved to see him as the WWF champion at some point.

DELETE: Triple H beat Randy Orton in 04. Cena's Second Title Reign. Not in it's entirety, he should have just gone a little longer before actually winning the title back. Ortons Current WWE Title Reign. Yeah, no one really wants to watch this Evolution Circlejerk.
ADD: Booker T WHC win. Even if it were just for 1 month, Booker should have gone over Hunter. Edge should have carried the title from New Year's Revolution to Wrestlemania 21 Cryme Tyme as Tag Team Champions.

Added: Booker T 2003 reign after beating HHH at Mania. Regal 2008 after he won King of the Ring he should gotten a world title run. Morrison 2011, he should have beaten Miz at Mania that year and not faced him at the Rumble.
Delete: McCool's unified Women's/Divas title Reign. She wasn't even the women's champion going into the Unification match, Layla was! Hogan's Reign after beating Yoko Jack Swagger's WHC reign

I'm just going to go from about 2011 because there's too many to change over the course of history.

Alberto Del Rio's run after Summerslam '11 - Punk should never have lost the belt. He could've still had the long title reign and just dropped it at Survivor Series rather than the Rumble.
The Rock's - Served no purpose other than to set up Cena/Rock II, this time for the title. I'm sure it made sense from a financial perspective. But as a fan I had no interest in seeing any of that. Have Cena beat Punk at Survivor Series and take it all the way until Wrestlemania where you have Punk vs. Cena in the main event.
Randy Orton's 2011 run as WHC - Undercutting Christian made no sense at that time since he was probably the most over guy in the company. Then they turned him heel to make sure Orton got cheered more (which still didn't always work). It was a mess. I know the IWC has turned on Christian in the last year but I still love the guy. Have Christian beat Orton clean a couple of times then move on to other competitors until the Autumn rolled round, then have him put over Mark Henry as the monster he was during that period.
Add:
Dolph Ziggler as WHC in 2013 - I love Christian as I said previously but he shouldn't, even though it ended up being a great match, have been the one to challenge Del Rio at Summerslam. They did the excellent double turn at Payback and Ziggler had got his feud with Big E done on RAW so it seemed that was the direction they were going. Instead they booked a tag match for him even though it was clear he should have been facing Del Rio. I'd have Ziggler win the title at Summerslam then retain in the rematch at NoC. He beats Van Dam at Battleground and then, as part of his show-off gimmick, challenges Cena to a match when he returns. At Hell in a Cell he retains vs. Cena via DQ when Sandow interferes and takes both men out. Sandow cashes in but fails as Dolph retains. Then at Survivor Series he loses the belt to Cena in a hard fought match. They shake hands after. This elevates Dolph massively.
Christian as WHC in 2012 - He returned at Over The Limit and I've just had him defeat Sheamus. I had no interest in Sheamus as Champion so I'd just have him lose it at Over The Limit and let Christian have another run since it would've been a bigger impact for his return.
Natalya as Diva's Champ in 2013 - They finally seemed to be building up Natalya which made me happy since I think she's the best wrestler the division has (although that's hardly an achievement). And it really felt like she should end AJ's run at TLC. AJ was doing nothing with the belt and honestly I'm still not over Natalya's failed run in 2010/11. At that time she was starting to get damn good reactions and people seemed to actually be into Diva's matches like they hadn't been for a long time. Eve then won the title in total nonsensical fashion at Elimination Chamber in 2011. It was stupid, but they seemed to have a chance to rectify that at TLC and failed. Instead AJ's probably gonna job to someone like the Bella's or Eva Marie at 'Mania despite the fact that none of those three have any skill when it comes to wrestling.

I don't know what I'd add... but I'd probably delete the Orton/Triple H/Orton title switch that happened in one night... and Ziggler's five second title win.
none of them amounted to anything that needed those title swaps except resume padding.

Delete: - John Cena and Rey Mysterio's reigns with the Interim WWE Title should've been stricken from the record books the very moment that Punk made his return.
- Cena's winning the WWE Title from Alberto Del Rio in 2011. Del Rio instead gets only one reign instead of two.
- Wade Barrett winning the IC Title back from Miz on the RAW after WM29. The IC Title win was Miz's only "moment" as a face. He should've held on to that title a little more, and then jobbed it to the next heel in a singles match rather than having to be shoehorned into a triple threat so that they could transition the title between two heels.
Add: - Alex Riley as United States Champion in 2011. Riley should've won the title at Night of Champions and then eventually dropped it to Zack Ryder in a face vs face program. Ryder/Riley at their collective peaks of overness could've been the "Team Edward vs. Team Jacob" feud that Stephanie wanted so badly.
Ryback as WWE Champion in 2012. Ryback should've won the title from Punk and then lost it back to him after a screw job.
*Not an addition but more of an extension. Daniel Bryan getting a decent length reign with The WWE Championship. Orton should've cashed his MiTB in for a legit match at either Night of Champions or Battleground and the screwjob could've been done there. Orton still ends up with the title without doing the Vacant stuff (someone will probably disagree with this just because this means no "Vacant is the Champion" meme).

Edge should have carried the title from New Year's Revolution to Wrestlemania 21 I would love for Edge to go back in time with the title. That would be an awesome gimmick.

Add: 1. The Rockers win the WWF Tag Team Championship. 2. Roddy Piper as a heel WWF Champion during the mid 1980s. 3. 3 Minute Warning as WWE Tag Team Champions (to follow Wild Samoans, Headshrinkers, and precede the Usos if they win the titles).
Delete 1. Alberto Del Rio's WWE Championship win (therefore, Punk held the title from Money in the Bank to Royal Rumble). Half the time WWE ignored the interruption in title reigns anyway. 2. Hornswoggle as Cruiserweight Champion 3. Hervina as Women's Champion
