One of mine was My Network TV's original format of doing English televisas every weekday night with a recap on Sat.
When I heard about this, I thought it was a bad idea and it was. That's why they changed formats.
What are others?

NBC executive: We should have Conan O'Brian take over The Tonight Show. Subordinate: But what about Jay Leno? NBC executive: We can give him a show in prime time five days a week. Subordinate: BRILLIANT!!! GENIUS!!! There's a great book called The War For Late Night that meticulously details for they screwed up every single step of that process. The Leno show was obviously a colossal disaster but the initial decision to go with a niche talent like Conan over the proven and still dominant ratings winner in Leno was the original brain fart.

DC's New 52 initiative was bound to cause more problems than it solved and everyone knew it. especially when the perceived "problems" it fixed were only problems in the minds of DC's editorial staff.

I had no issue with them making a Lone Ranger movie a few years ago, I didn't at the time understand why they was ploughing $250m into it (not forgetting advertising budget), I wasn't surprised when it didn't end up as a money maker for them

I thought McDonalds Mighty Wings were doomed long before they hit the market. From the start it was the wrong sort of product. McDonald's seems to thrive by offering cheap food. Cheap wings suck... and if you want premium non-cheap wings you aren't going to go to McDonalds.
That doesn't even get into the whole "wings and beer" combo that would be non-existent.
That was another one of those moments where McDonalds forgot who they are.

There is virtually no nostalgia for The Lone Ranger, so if you want to do that as a franchise I'd think the smart thing to do is to start small. Maybe go ahead and don't cast Johnny Depp and save some money for one.

Card Sharks is coming back...but with wacky video clips and barely resembling the old show.
When the US version of Temptation was announced and boasted of "funny pop culture questions," I knew something was up. Little did I know that would be the least of the show's problems.

Then having stuff like Batman and Green Lantern's entire history apparently still happening which is entirely impossible in the 5 year time frame they were working with (and again things like Blackest Night not working when none of the people involved died and half of them didn't even exist at that point) It should have been obvious to anyone that it was a bad idea. The initial error NBC made was in not having the forsight let Conan go to FOX in 2005 when he was renegotiating. It's easy to forget but from around 1998-2003 with the brief exception of the lost 9/11 Letterman shows Late Night was the hot show. David and Jay were mostly on auto pilot. The Daily Show didn't find it's footing totally until Bush's second term and Late Show with Craig Kilborn pretty much sucked. But Conan' s 12:30 show, like Letterman's before that was niche. NBC clearly want him for that timeslot, not for The Tonight Show. So when Conan demanded his getting TTS in that contract and it was agreed to NBC should have realized they were f'd. Because Jay wasn't Johnny. Johnny moved being off. Jay worked all the time. Jay still tours as a stand up. Johnny took a bunch of time off a year and then retired. Jay wasn't going to do that. Plus he had a giant contract with a massive buyout clause. So really NBC should have just let Conan walk in the first place.

They really must've thought Depp was guaranteed money...
