I think the reaction is nothing more than basically a huge number of dudes and even some women to be honest, get really weirded out about periods and anything to do with them. So the idea of period blood being put on someone is something they can't handle. To guys it's completely disgusting and uncalled for and to women it's low class and you shouldn't even mention your period let alone do something like that.
The vast majority of society have it in their heads that periods are like the most disgusting thing ever so this strikes a nerve. Hell to a lot of people they'd rather have shit smeared on them than period blood, it's that bad. In reality this is not as bad as a ton of things WWE has done but the fact that is associated with a period is why it's setting people off.

FWIW I don't look back on any of that fondly. Especially the dangerously concentrated piss that Arn had dropped on him. I find it all insanely lowbrow and embarrassing whenever I get into a conversation about wrestling with a non fan. I don't need wrestling to be 100% Okada vs Omega levels of serious. But bodily fluid related humour hasn't worked for me in a long time. You're way off if you think I'm in favour of any of the stuff you referenced. Think of the best angles you've ever seen on WWE TV. How many of them involve semen, piss, shit or menstrual blood as a focal point? There's a reason the best TV shows written don't involve this stuff. Just because it exists doesn't mean I enjoy it on WWE or any wrestling show. Who said YOU had to be in favor of it for it to be okay to be a thing? If that's what you got out of my post, you completely misread it. At no point did I say that. What I said was it existed. It happened. It has happened off and on for decades. Whether I like it or not, whether or not you are in favor of it, that's not relevant to the point. It exists. It has existed. It is part of the business. Whether you or I approve, it has become part of the business for decades. To do the "Not in MY wrestling" thing is incredibly near-sighted given the scope of the business' history. I've never said it wasn't part of the business. I said it was lowbrow crap. Just because it is part of wrestling doesn't mean I enjoy it. I'm talking about what interests me in wrestling, one person's opinion. Nothing more or less. I don't give a shit if it existed in history. It was garbage then and it's garbage now. While I agree that has something to do with it, there's also a factor of, if it wasn't gimmicked, and it was a real used tampon with real period blood, that is a HUGE biohazard. Most places have specific procedures to deal with it, because blood is a contamination hazard to anything it touches.
