Seems like Stevie Ray did this for Booker T. in 1998 WCW with the TV title. Booker was hurt or something so Stevie started defending instead, although I don't think he actually had permission to do it. Jericho beat Stevie for the title if I recall, so these guys are involved in multiple scenarios like this. At first, he did not but then eventually after several weeks he had papers that said that Booker gave him power of attorney or something like that and eventually WCW just let him have it until he lost it to Jericho.

Cole took someone's place in a tag title match before. Wanna say it was Eugene. Looking it up, it was Jonathan Coachman subbing in for Eugene on a house show to defend tag titles held by him and William Regal, because Eugene had just blown his knee out the week before. It only went one match because they lost the belts to La Resistance in it.

Chikara did this fairly well recently, with Mark Angelosetti getting injured and his partner Dasher Hatfield defending the belt in his stead. Naturally, it lead to Angelosetti coming back, and Dasher turning heel by saying that he had to "Prove he could earn the belt back" from his surrogate. Neat stuff.
