I know that a lot of horror movies have their villains try and look scary and it just comes off as looking really corny, but, the scene in the Conjuring 2 where you first see that goddamn nun is legitimately one of the most tense moments I've ever seen in my damn life.
Like, I'm not really a horror buff, so I only really watched that scene on YouTube and it still had my damn skin crawling, I think it's because, from a distance, Valak in that nun garb is almost human looking and, yet, at the same time the character looks eerie and horrifying, and that's before you even see the teeth.
I think that, honestly, Valak's look in The Conjuring 2 is one of the coolest horror villain looks because of how damn simple it is, just this ghoulish, haggard woman in a nun outfit is all you really need to inspire dread.

I don't know about how "cool" he looks, but Leatherface literally cuts off and wears other people's faces after murdering them with a chainsaw. That is f***ing terrifying.
And while Freddy and Jason and Michael Myers are visually iconic, they were unstoppable almost supernatural beings. There can literally be Leatherface type psychos out in the world right now, which makes it even scarier.

Jigsaw
The puppet gets the most attention, but Jigsaw himself looks as sinister as hell. Sure he's just an old guy, but his stare, mannerisms, the fact he looks pretty ordinary make him cool and frightening.
I also have a fondness for Otis Driftwood from The Devil's Rejects. He looks awesome.

A couple I thought of would be:
Crate Monster from Creepshow; It's an anthology but this deadly creature is an amazing visual. It's the first thing I thought of when I read the title of the thread.
Pumpkinhead from Pumpkinhead; The story is ok but the demon created by a father's need for revenge whilst in a state of absolute grief is a stunning bit of cinema.

Horror movies have so many unlikely success stories, but that a huge fixture of pop culture at one point, and a remaining icon, is a burned pedophile with a claw glove and shitty sweater is still wild. Robert Englund, great effects work, and an awesome prop really came together for something special.

Pinhead is another one. Simultaneously terrifying and regal. And Doug Bradley put so much into his performances. Even in the crappier Hellraiser movies, you could always count on Pinhead to be awesome.

Friday the 13th Part VII is one of the weakest of the series, but also the most terrifying Jason has looked without his mask.

One that hasn't been mentioned that doesn't get enough love is Ghostface from Scream.
Some of those movies sucked, but that was a great look for a slasher villain.
