Randy Savage getting bit by the cobra is in the opening minutes of the Survivor Series 1991 PPV on the network.
As for the incident where Quake squashes Damien, I don't think there are any Superstars or Prime Time from that time period up on the network just yet, they talk about it on the 4/27/1991 SNME before the big battle royal but that's about it so far.

Yes, that's right. The angle aired on Superstars earlier that day, so the short SNME clip of it was the first time it was shown uncensored. I believe it was also not censored when they aired the entire segment on PTW the following Tuesday.
