That new awful show on TBS, has a 47 year old playing a 15 year old That is a stylistic choice if I am not mistaken. They have done that with the Hulu show Pen15.

The Last Kingdom has a bizarre approach to charachter ages. The best being lady Aelswith and her daughter being played by actresses of the same age. TBF she does it really well.
In the books Harry Potter's parents died at 21. Snape, Lupin, Sirrius and Wormtail should have been 31 when Harry started school. Aging them in the movies up worked out though.

John Mills in his 40s playing Pip in Great Expectations was pretty laughable to me.

Stargirl's a fantastic show, and they've done a great job with the casting, except for the guy playing Icicle Jr. He's 27, but that cat looks mid 30s, and he's supposed to be in high school.

The original Halloween cast. None of them look like teenagers and first time I watched it I was sure they were supposed to be college-aged.

Too Short a Season remains Trek's worst attempt at putting a young actor in old age makeup; so much that perpetual old man duck face is the thing I most consistently remember about it:

Now I'm not sure if this counts. But to me, the 1967 Spider-Man sounded too mature for the age Peter Parker's supposed to be.
The 1980s series and up got it right.
As an off-shoot, Olan Soule as Batman made him sound much older than he should be.

Sandra Bullock looks really well for her age (I think it's 55+ now) but she's been playing 35 year old women in movies for 20 years, and it stopped being believable a decade ago.
One that worked: Elijah Wood playing a 51 year old Frodo while looking 30 years younger sounds bad on paper, but it really worked well in the movies having all the Hobbits close in age. If I remember, book Pippen was a borderline adolescent hobbit at like 29ish. Frodo had like 10+ years on Sam. And Merry and Sam were fairly close in age.

Remember seeing a review for that old Babe Ruth biopic they made when the real Babe was on death’s door.
And at one point the the middle aged actor they portrayed him also played Ruth as a 13 year old.. it was quite something
