Honestly, those adult romances kinda suck anyways outside of the ick factor tbh. The romances between the other Phantoms are much more natural and interesting. She’s not an adult, but I found the romance with Hifumi Togo more interesting than any of the Phantom Thieve girls. Ann’s a close second though. Everyone likes Makoto, but she may be my least favorite of the team members. Too bossy for me. I’m assuming everyone likes Makoto because she’s a brainy student council president by day, badass biker chick by night. She’s Sandra Dee, essentially. Well, that and some people are into fit, petite brunettes.

She really rubbed me the wrong way during the scene where she asks Joker what he thinks about making a certain person the guest at the school festival and basically blows your response off if you disapprove.

In Royal, if you're smart with her skills and trips to the Jazz club, Makoto can become a complete beast in the endgame. I feel like the person who really gets a glow-up in Royal is Ann. Between her new abilities and some of the new items (especially the Salvation Crown), if you use the Jazz Club to teach her Spellmaster she is an insanely good support player.

So far I've just done Ann and Makoto for romances. I want to try the other ones for completion's sakes, though the adult-related ones creep me out. I'm not sure what the age of consent is in Tokyo (it's apparently 14 as baseline in Japan at the national level, but IIRC most prefectures and cities have a higher age, usually between 16-18), but even if they are legally consensual, the power dynamics involved are a big turn-off. And Futaba feels ace-coded (and I personally see her as more a gremlin little sister than a romantic interest) so I'm not really looking forward to that either.
The Yoshizawa one is also weird because it's stuck at 5 until the third semester. So while all the other ones advance over the course of the game that's just in stasis from July until January. I want to do a playthrough where I commit to that one but between that and how rare hangout opportunities are with her, I usually just go "forget it" in favor of one of the other characters lol.

Ann Hifumi Futaba Kawakami Everyone (At once)
Hifumi and Ann are tops for me. I tried to do an individual play through for every romance so I could get all the Valentine chocolate, but there was only so many times I could stand to play the game before I got bored.
My romance choices in each Persona game:
3: Mitsuru 4: Naoto 5: Ann/Hifumi

The only substantial differences between vanilla and Royal narratively are that Royal introduces two new characters, Kasumi and Maruki, who appear throughout the story and then heavily factor into a new post-story set after the original final boss. The broader story is largely the same until that point.

I suppose that I might still go on dates because it’s weird otherwise, but which confidants should I want to focus on maxing out besides my main party (honestly, I’ve stuck to almost exclusively using Makoto, Ann, and Ryuji, with that last one so I get driven less insane by having him in my party altogether). I maxed out the teacher because, despite her storyline being all of the ick, her perks were awesome and just kept getting better. I also maxed out the doctor for similar reasons (the SP patches and medicines are MUST-HAVES). Are there any others that I should focus on?

Dr. T is an alien You're braver then me. I got so lost on who to take out or when to get skill bonus I couldn't do the game without a guide. So much so the first time I played I gave up 6 months ago after the guide I was using turned out to have updated the first three months and not the rest. So you didn't have the stats needed to do what it wanted went it went back to the updated stuff. I was so unsure what to do and just gave up instead of thinking for myself. That being said I got the itch to try again and restarted the other day. But still not thinking for myself on the stats of relationship side, found a guide that gives me all that and says nothing about any battles in fact when it comes to that side it just says when to enter and stuff like clear route today or get to this part on that day and any types of Persona I might need to pick up for the relationship stuff. The fights, where I have to go, what I do in there, where the will seeds are I'm figuring out myself. And I have learnt I hate negotiations

In terms of story, an entire new "third semester" scenario. It introduces a couple of new major characters (Kasumi Yoshizawa, a first year student and potential love interest for Joker, and Takuto Maruki, a counselor hired by the school in the wake of what happened with Kamoshida who is voiced by the late Billy Kametz). Kasumi and {Spoiler}Akechi, is in his full blown black mask outfit with Loki as his Persona, and acts as a slightly deranged and snarky navigator for a bit. are new party members in this scenario. In order to get to play it, you have to get Kasumi's confidant level to 5 and Akechi and Maruki to as high as they can go by a certain point in November, I believe.
In terms of gameplay, a new section of Mementos, a new Palace dungeon, some new minigames and hangout spots like the Jazz Club, and a shitload of quality of life improvements like gun ammo refreshing to full after every battle, Showtime combo attacks, revamped Confidant availability, etc. All the costume/music/Persona DLC from Vanilla P5 is also included for free in Royal.

I always forget about the bonuses from the Jazz club, need to use it more often
And finish Strikers, STILL need to do that. Damn I'm slacking. Think it's the gameplay, I suck at the Dynasty Warriors kind of games.
