Bionic Commando if just for the exploding Hitler head With that being said, while this one was given an SNES port and was edited, "Total Carnage" would be M-rated if it was like the arcade version. I saw some videos of the SNES port and there was green blood instead of red blood. Yeah, like that is any better. It's like that for "Contra: Shattered Soldier" where there was green blood and it was given a T rating whereas the sequel, "Neo Contra," was rated M because it had red blood. Oh, and back to "Total Carnage." The true villain was actually Hitler and it was his gigantic head. Now that I think about it. I recall that the arcade version of "Smash TV" was gory as well but the NES port had more of an explosive effect sprite when enemies were killed. The Genesis (Mega Drive) version actually had blood. If the NES port was like the arcade version, it would definitely be M-rated.

Maybe Dr. Jekyll & Mr. Hyde too. All that gratuitous poking with your cane would be enough to elicit an M-rating.
