So I was at a WWE Live show in Glasgow on Halloween and there was a match where Santos Escobar fought Karrion Kross.
It was a decent match, botch guys know what they’re doing and Kross dominated most of the match until he was about to hit the forearm to the back of Santos’ head. Santos ducked it, schoolboyed him and got a win.
Nothing notable there, but here’s something I’ve legit never seen, or at least don’t think I’ve ever seen, happen. Santos left the ring while Kross fumed at the ref, saying it was a two count. He then chased the referee out of the ring and then chased him down the ramp and to the backstage area.
Now I’ve been watching WWF/WWE for over 30 years and for the life of me I can’t remember that ever happening before.
Has it?

It seems like a fun house show bit that would get a good laugh from the crowd with the ref running in terror. I'm guessing it's happened at times on TV over the years too.
On the Raw with the phantom title win for Jericho HHH was furious with Earl Hebner. I can't remember if he literally chased him to the back though.
Not a ref but ring announcer Lilian Garcia was chased around the ring one time by again HHH when she made a point of introducing him as the former world champion the night after he had lost the title which was pretty funny.

Earl Hebner once ran from the ring into a waiting car that sped away from the arena.

Yes, Karrion Kross loses a lot.
