Mortal Kombat II, since I had a SNES and the first one wasn't M-rated, best I can tell. First one is one of the reasons the ESRB ratings were invented, yeah. Though the SNES version might've been rated T if it did get rated because of all the Nintendo of America-mandated censorship with blood becoming a gray "sweat" and the fatalities being really toned down. Although I will argue that, while the normal fatality for him in the first MK is iconic, I think the SNES Sub-Zero fatality fits the character's abilities better.

I guess Ultimate Mortal Kombat 3 when I was like 7. Played the first game before that but I don't think ratings were really a thing yet for that one.

No clue what was teh first legit one,by the time they started the ratings I was 19 or so.
But stuff that came out on a home console that would have gotten the MA? Guessing Bachelor Party on the Atari 2600. Was late 80s and someone found a copy in a lot of Atari games at a pawn.
If you want to include PC games there was some text only adventure game that got copied and passed around jr high. Was fairy tale/fantasy based. Just remember one guy figured out how to talk Goldilocks into taking you up to the bedroom. And the Three Bears walked iin and shot and killed ya.
And IIRC Night Trap was never banned in the US. What a shitty game too. The Sega CD had almost nothing worth playing for the first year. Night Trap was one I grabbed just to have a new game that week. Sold it off a few years later.

Freezing then shattering is very Sub Zero-y. Though Bi-Han is a dick so maybe the spine rip is in-character for him.

I'll echo what others have said and say mine was one of the first two Mortal Kombat games. Though the first M rated game I really felt like I was doing bad things was Grand Theft Auto 3 and I loved it.

Doom 2 at around age 5.
What's funny is that my cousin had a mod that let you kill the Power Rangers and they thought it would upset me, so they tried to explain that they weren't the real Power Rangers but bad guys in disguise. I didn't care though and just asked them how to use the tooth brush mod so I could kill them.
