Either Mortal Kombat 2 (I can't remember if it had an M rating at release, but it did get retroactively M rated in later re-releases) or Doom 2.

I guess Mortal Kombat on the Sega Genesis would count? It was MA-13 or whatever at the time. It's also the greatest game ever made. Not like Donkey Kong, Donkey Kong sucks!

Either Doom or Mortal Kombat. Just the amount of controversy surrounding those games made any child want to check it out just because of that.

I guess "Splatterhouse 3". Was great fun, but I never beat it. Actually, it's hard to tell, because that was the time I was introduced to many Mega Drive and Mega CD games not meant to be played by elementary school students.

I'm assuming the Leisure Suit Larry games we played back in the before time would have been rated M/AO if the rating existed then. The first "Actually Rated M" game was probably Doom.

Ultimate Mortal Kombat 3. My uncle owned a copy.
I was quite disappointed the first time I came across the classic MK3 arcade game in Canada and found it ninja-less.

Mortal Kombat II, since I had a SNES and the first one wasn't M-rated, best I can tell. First one is one of the reasons the ESRB ratings were invented, yeah. Though the SNES version might've been rated T if it did get rated because of all the Nintendo of America-mandated censorship with blood becoming a gray "sweat" and the fatalities being really toned down.
