Inspired by the other thread about movies.
Personally, it'd be Beyond the Valley of the Dolls. I know its intended as a parody but the "Manson" scene just disturbed me.

An Andalusian Dog. Anyone who's actually seen it will know why.

I really love disturbing movies. So I'm not sure if they count as disturbing me, if I like them. I'll have to mull this one over.

Salo is literally the only movie I've ever felt uncomfortable while watching. Only the first time, though. Then I read the book. Now I'm a de Sade fan. So I'd say Salo too.

Imprint from the first season of Masters of Horror. A severely f***ed up, twisted piece of work that, even as a horror fan, I have no interest in ever seeing again.

Cannibal Holocaust Funny Games (original Austrian version)
There is a French movie that I forget the name of. It's about lonely women whose husband is out serving in the military. Anyway she takes in a female backpacker who has some issues in her past. Anyway the film has one of the most disturbing endings I have ever seen.
