All of Dead Space.
Mr. X in Resident Evil 2 bursting through the wall in the candle room where you have to light all three candles. The first time it scared me. However, after you defeat him, you walk out and down the hallway past the hole in the wall he burst through, and he bursts through into the hallway to fight you AGAIN! AHHHHHHH!

The giant polar bear in Crash Bandicoot 2. It's a chase level, and the previous chase levels involved you running away from boulders so at 5 years old I assumed that it would be another boulder chasing me. Nope, instead it's a giant polar bear with huge *** teeth that made me **** myself and I walked into a pit. It was one of the most traumatising things in my childhood. I had to get my dad to come in and play the level for me because of it.
Also, Crash's death animation when you get stung by the bees. Don't know why it freaked me out so much.
When you're in the sewers in Arkham Asylum and you try to get into Killer Croc's lair, only for Croc to LUNGE at the door with a close-up shot of his face made me jump.

Everyone says the dogs in RE1. That was just a jump scene. What really kept me up at night was the first zombie you see...the way the camera zooms into his face, the way you don't know at first what it is until it slowly turns around and stares at you with those souless eyes, the realization that you're pretty much f***ed. I think RE1 was the first video game that gave me a nightmares, because of that scene. THIS! That f***er creeped out me out. Playing at night with the lights off completely by myself didn't help. But you know what, it's moments like that that stick with you for years. RE1 was really the only game that ever scared me. Games like Dead Space, while good in their own right, try a little too hard. That's just my opinion of course. And while the dogs were a shock moment, it still surprised and scared me, so I'll count that too.

Kind of pointless since the entirety of Resident Evil 2 scares me, but the part where you walk through the hallway and the zombies grab you through the window scared the living hell out of me! I hateee that part with a passion.

I never found the original 3 Resident Evil games to be scary. Some of the jump scares would get me but that's about it. Great games, just not scary IMO. The fast "zombies" in the newer games are scarier to me.
Something about the dam level in the 1st TMNT for the NES used to scare me. I'm thinking it was something like the music changing when the timer on the bombs started getting close to 0. When I got to that part of the game I always knew I was going to die. Always. I never even came close to beating it. Then again I was like 5 years old, so I guess it's understandable.

I forget if it's a machine gun or a shotgun in Bioshock, but you pick up this gun in this calm, empty room...then the lights get hit and there's splicers all over the place from out of nowhere. Is that the flooded basement in Fort Frolic? That made me jump like a little bitch. Also earlier in the game, where you pick up an item on a table and turn around to come face to face with a Houdini Splicer. Damn near wet me pants.

I think the RE dogs were scarier in the Gamecube remake. You think you're prepared for them (if you played the original), but then...they don't appear. Then later on when your guard is down and you're backtracking through that room again...CRASH!

I'll second the Mr. X Kool-Aid Man thing from RE2, but also add in whenever he grabs the railing on the top floor of the police station and lifts himself up in front of you.

I'm arachnophobic, so the first time i saw one of those huge f***ing spiders in Resident Evil i almost crapped myself. You ain't f***ing lying, man. When the remake came out, I was a lot older and had mostly forgotten the experience. I made the mistake of actually buying it. A ways into the game, you walk into a room. It looks empty, harmless enough. Then, shit got real. You take a step in and the camera angle changes to show one of those f***ers on the ceiling right on top of you. It dropped down and I don't even know what happened after that. It's all foggy. I couldn't play the game after that. I just couldn't. Sold it the next day and wrote "F*** those spiders" in the instruction manual. Playing Resident Evil 4 recently is NOTHING compared to that. My 8-year-old nephew could get through RE4 without getting scared like I did with RE1. On the subject of scary moments, that 3rd Scarecrow encounter in Arkham Asylum was just cruel! That was one of the most legit TERRIFYING experiences I've ever had. Why? They made me think my PS3 was dead. I almost had a panic attack. I had to pause the game and collect myself in order to continue. I kinda wish the videogame in the "Stay Alive" movie was real. That game looks like a complete nerve-killer. Being based off a true story adds to it. Watching that entire movie, I just kept thinking "This bitch actually did stuff like this." A game based off of "The Blood Countess" Elizabeth Bathory would've been very, very interesting.

Oh my f***ing god Redeads. The insane ones from Wind Waker were freaky but the OoT ones just scared me shitless. I had my friend get the Lense of Truth because the well was just way too much.

The Stomping Plains from banjo-tooie. I did not know my brother played before I did so I did not see the intro movie when you walk in here. Next thing I know a GIANT FOOT kills me in one shot. >_> Almost any Scarecrow moment in Batman: Arkham Asylum. The 1st one did not creep me out until you leave that room and enter the room you just left. The 2nd time did not creep me out at all. The 3rd one... That was just cruel.
