Fatal Fram/Project Zero 1, so many times, but the worst was hearing a guitar (or some other instrument) playing in a large 2 floored room...heading up the stairs and seeing a shadow behind a screen playing this noise. Stepping forward cautiously for the music to stop suddenly. The music stopping made me and a few friends jump and shout in "oh crap!"ness.
Project Zero 2 - whilst battling ghosts in a room to pick up an item I happened to catch something outside the window swinging. Then it dissapeared...when I'd dispatched the damn things I couldn't see anything...picked up the item, walked back, checked again and saw a woman hanging from a tree swinging with the breeze, lifeless, took the creepy picture for the points. Left the room, but then every time you entered a room with windows to the outside she was looking in directly at you. Not moving, not attacking. Just staring. Utterly unnerving.
I'm gonna stop posting any details now as you should, if you love horror games, have blasted through the Fatal Frame/Project Zero series. They're 20 times more frightening than Silent Hill 2 or 3.
Also whoever mentioned the bathroom scene in Eternal Darkness, God yes.

Eternal Darkness bathtub scene. Can we also count the blue screen error? I mean some people were really scared that their Gamecube were broken. I really wonder if some people turn it of before revealing it was another fake.

The Stomping Plains from banjo-tooie. I did not know my brother played before I did so I did not see the intro movie when you walk in here. Next thing I know a GIANT FOOT kills me in one shot. >_> Yeah, but come on man, it's called the Stomping Plains, and there are giant footprints everywhere. What did you expect was going to happen?

I always hated going into an Oblivion Gate...and I was teenager, too. It says a lot cause nothing ever really scared me or made me nervous in a video game before but Oblivion nerved the shit out of me and I was always trying to get out of there as quickly as possible.

When I was a 5-year-old playing Super Mario Bros., Level 1-4 terrified me. The music scared me shitless and Bowser made it worse.

The bad ending for Chrono Trigger if you happen to get killed by Lavos at anytime. If you've seen it then you know which one I'm talking about.

I was scared s***less haha! My pick from that game would be either the giant spiders, or, jumping ahead, in RE2, the first time the Tyrant appears in game B made me jump.

Pretty much a lot of the moments that put me on edge or have scared me have been mentioned in the thread. I don't know if I could call this a known moment or not, although I am sure it's happened to some people.
I was playing Resident Evil 4 and when you're locked in the freezer with the Regenerator. I equipped the infared scope on the sniper rifle, ran around trying to get a good shot at the plagas, and I figured "Well, let me run around some more and create enough space."
Little did I know that they can run too and when it started running, I literally took a gasp and ran as well, then when I stopped to get another shot IT WAS STILL RUNNING. Imagine the image you see is a Regenerator right in your face through the scope. It proceeded to throw its arms around and knock me down.
I remember very vividly jumping from the chair and yelling "HOLY SHIT!" It scared me so bad that after I killed it, I took a break.

You fight Mr. X in some room, then once you beat him he comes busting through the wall in the next corridor!
Totally not expecting it and damn near gave me a heart attack.

Oh man, how could I forget this one:
Kingdom Hearts, the Atlantica level, with the giant f***ing shark crashing through the ship while you're in it! I had to pause the game as it was happening because I was actually having a minor panic attack, and it took a great deal of courage to keep going after that. It's that, as well as a few other frightening instances, that makes me very reluctant to play that game over again.

For some stupid reason, my reaction was to jump up and turn the power off before it got a chance to delete them all. Which was completely stupid. The moment that gets me was in Resident Evil 4. When you first have to go down into the Sewers and meet those Novistadors, or whatever they're called (those bug like things). Before you actually fight one of them, there's a bit when you are running down a sewer and you can hear something really loudly running up behind you. You turn around and nothing is there and there never was anything there. But the way they mixed the sound makes it sound so terrifying the first time through.

It's funny that you mention that, because I have a friend who went through that exact same situation. We still give him crap about it to this day. Anyway, I have a few moments of my own: When I was about 9 or 10, I had a demo disc with MGS on it. I literally could not play it unless someone else was in the room watching me play. It was so nerve-wracking that I would freak out and exit as soon as it started. Those "blood" levels in Max Payne. Darkness, blood, satanic crap, and the sound of a baby crying? *heart attack* I borrowed Twisted Metal: Black from a friend in 8th grade. Thought it would be fun to play as Outlaw in story mode, since that was the only character I was familiar with. Watched about a minute of the first cutscene, had the crap scared outta me, and quickly turned it off. I was so scared that I didn't even sleep that night. I just lied in bed and started at my curtains to make sure no one came in. Currently, playing through F.E.A.R. 2. DAMN is this game scary. I love it though. The otherworld or whatever it was called in The Darkness. It's like, a dark world that resembles a WW1 battlefield, complete with soldiers. But there's some twisted shit going. One thing I will never forget is that I was going through the trenches and saw a soldier sitting on the side. Now mind you, he wasn't an important part of the game or whatever, he was just an NPC. He proceeds to pull out a gun, stare at it, put it to his head, pull it away, start crying, rocking back and forth, talking out loud, etc. Essentially contemplating suicide right in front of me. Then he finally pulls the trigger and offs himself. A few seconds later, he gets back up and goes through it all over again. As someone who has battle depression my whole life, it was something that really freaked me out. They really went all out to create a f***ed up universe in that game. Can't wait for the sequel! I have more, but this post is long anyhow.
