Whether it be TV show to movie, cartoon to movie, book to movie etc.
For me the best two are Viggo Mortensen as Aragorn and Alan Rickman as Severus Snape. When I watched them it was like actually seeing the characters come to life. They completely nailed it. They were perfect.

Patrick Stewart as Professor X.
Also pretty much half the LoTR cast: Gandalf, Saruman, Sam, Elrond, Aragorn, Theoden, Eowyn, Boromir/Faramir (sans black hair)...you not ask for a more perfect cast.

What about adapted from real life? Because Daniel Day Lewis as Abe might be a contender

Robert Downey, Jr. as Tony Stark/Iron Man. That's because Robert Downey Jr. is literally Iron man, went through similar things to Tony Stark battled his demons, over came them and got rich by following in his families footsteps

Came in here to post Alan Rickman as Snape. Maggie Smith as McGonagal was another very well cast role.

Mark Ruffalo as Bruce Banner He was a little too spacey for my tastes, preferred Norton.

J.K Simmons as J. Jonah Jameson Definitely one of my choices. He brought that character to life not as an actor's take on the character, but as the character jumping right off the page and onto the screen. Absolutely fantastic. Another of mine is Michael Fassbender as Magneto. For every god awful thing Fox has done with their Marvel movies, this made up for most of it.

All casting in the Marvel Cinematic Universe Well, let's not be too generous. Their casting of Black Widow really sucks.

John Goodman and Elizabeth Perkins as Fred Flintstone and Wilma Flintstone.
The rest of the casting was blah, but those two seemed right.
Keep in mind I haven't seen "Viva Rock Vegas" to compare.

Aside from most of this thread, the only one I can think of is Cary-Hiroyuki Tagawa as Shang Tsung in the Mortal Kombat movie.
